Question: Hi, can I answer your health question? Please type your question here...
My 18 year old daughter was put in the hospital in November 2017 after a myocardial infarction. The cause was myocardtisi ruled as viral. She was released after a week. Aimce then she has gone to the ER and doctor many times with chest pain and she dos have sinus tachycardia. After months of this we took her to a new carsiac dr and they checked her troponin which was slightly elevated. In November troponin was above 17. The new dr also did a cardiac MRI which showed no change in heart since November. Still same amount of myocarditis. The first dr never did these tests and specifically said her heart was fine. Is this normal to have myocarditis this long??? Her EF was above 55 last time first dr checked few months ago

Brief Answer:
I would explain as follows:

Detailed Answer:

Hello,

Welcome and thank you for asking.

I passed carefully through your question and would explain that not rarely acute myocarditis, due to several causes such as viral, microbial, physical,
chemical, etc. agents, may lead to excessive immune response, which not rarely may become chronic.

In such case, due to the excessive inflammation within the cardiac muscle, autoantibodies against several components of cardiac myocites are produced.

It is because of these cardiac autoantibodies, that cardiac inflammation keeps going and leads to cardiac tissue damage with subsequent increased troponin levels when assessing the blood tests.

Coming to this point, besides a careful evaluation of her cardiac structure and function by means of cardiac ultrasound and cardiac MRI, it is necessary checking the levels of inflammatory response by testing:

- complete blood count with leukocyte formula
- PCR, ESR.

What is more important is checking for cardiac autoantibodies during serology testing.

If detectable and increased levels of cardiac autoantibodies are concluded, then it is recommended to pass through immunoe modulation and immune supression therapy.

These measurements will control further cardiac autoinflammation and will prevent on going cardiac muscle injuries and will maintain that way normal troponin levels and normal cardiovascular conditions.

You should discuss with your doctor on the above tests.

Hope you will find this answer helpful!

Kind regards,

Dr.Iliri
