Posted on Sat, 24 Jan 2015
Question: hello i had my circumcision 9 days before and skin and mucosa was cut using a 980 NM laser

,suturing of mucosa with skin was done ,

Problem area :-

1) i have huge swelling below glans the swelling is as big as glans themselves
2) and have some watery discharge ( with light smell - say fish smell) ..

Questions in my mind :-

I want to understand
a) Is this normal ? or is this swelling not normal ?
b) Which antibiotics should i eat ...please prescribe as per availability in XXXXXXX .I am ready

to eat antibiotics as a precautionary measure
c) How do I distinguish between normal swelling and infection

Other indicators :- i don't have any pain , the urination is very normal , i am not wearing any

underpants , .i am basically resting whole day so not much movement in the area , I don't have

fever and everything else is normal , I don't have a compression bandage except some tight

cotton seal around the stitches with instructions to leave the seal as it is for 2-3 weeks .I

don't have any fever

Medicines currently prescribed

1) augmentin 625 course was given for first 5 days only- over . The course is over now
2) To wash operated area with Betadine twice a day which I am doing
3) I am told by Laser surgeon to avoid putting water on operated area till the time healing happens so I am not doing that

In the pictures you can see

1) Swelling below glans
2) Betadine solution everywhere

Brief Answer:
Asymptomatic not be of concern, will subside in few days.

Detailed Answer:

Hi Sir,

I understand your query and concern.

I have gone through all your uploaded images as well.

As per your clinical description, this seems to be a normal post operative fluid accumulation at your galns.

You need not worry since there are no active signs of infection or inflammation or urinary disturbances.

You need not take added antibiotics.

Augmentin and Betadine wash are all just enough.

Tab Serratiopeptidase when taken under doctor's prescription will help reduce the swelling by resorption of fluid.

This swelling will subside in 4-6 days time .

Drink plenty of fluids.Take care of the operative site.

Avoid trauma to this area.

Consult Your doctor only if the swelling does not subside in 7 days.

Post your further queries,if any.

Thank you.

Brief Answer:
You received right medications.

Detailed Answer:
Hi Sir,

I understand your query and concern.

I have gone through your recent image as well.

You have chosen the right medications.

Also if possible you can take Espidase D or Lornidase which are more effective than plain Espidase next time when you purchase.Continue these medications for 5 days.

Betadine wash as usual.

Please note recommended drugs have to be taken only under strict medical prescription

Brief Answer:
Espidase alone is sufficient at the moment.

Detailed Answer:
Hi Sir,

I understand your queries and concern.

The swelling is expected to subside in 2 weeks from here on.

Need not take augmentin 625 again.
Espidase 10mg TWICE A DAY is all that is enough at the moment.

Consider a culture and a sensitivity of the fluid in the swollen area, since a slight foul smell is noticed by you. Need further antibiotics only if infectious organisms are detected on culture.

This will help us to detect any infection at the earliest as well as focus on the required antibiotic regimen.

Yes icing will be pretty helpful in decreasing any associated inflammation.

Consider wearing cushioned Jockey sport underwear.

No specific home remedies available to reduce swelling in this area concerned.

Yes definitely you should wait for the swelling to go down before sutures are removed.

Thank you.
