Question : This question is somewhere in the realm of ridiculous and wtf? Okay, there is a FB XXXXXXX going around asking if you would "drink the semen from a condom for a million dollars." My response to this was "yes." It is my understanding that 1. It is unlikely to contract HIV from swallowing semen bc as long as there are no cuts in the mouth- and that the acid in the esophagus and stomach kill the virus 2. HIV cannot live outside the body in semen for extended periods. My greater concern being a latex allergy. So my question is, if a person was to come to you and explain that they were paid 1 million dollars to drink random semen from a condom and wanted to know what their risk to exposure was and wanted a prescription for PEP, what would your response be? Would you write the prescription and call their insurance company to have it put through if they needed a prior authorization for the medication to be covered?

I read your question and i understand your concern. Semen can carry both HIV and hepatitis and many other STDs and can be contracted on ingesting semen as that is valid enough fluid exchange between semen and your bodily in GIT.
With non ulcerated mucosa the chances are less but still there. One out of 100 cases will get such complications in average.
As far as the latex allergies are concerned if you are already known case of latex allergy then this would be a disaster. If not then odds of having a latex allergies are also few in thousand.
As far as someone comes to me with this , i am bound to serve the patients no matter how they get it. I will give them a prophylaxis and treat them to be on the safer road and will advise them to void such acts.
Regarding the insurance issues that is some thing an insurance company officer can guide you well. My job is to treat and that i will do like any other doctor.
