                                            Question : Already a patient of rheumatoid arthritis from last 30 years. Continuing on the medicines listed in current medication. It was good since last 8 months but it started paining suddenly at below places :
Unable to lift the shoulder
There is pain and swelling on the fingers and wrist
Knee pain
Pain in the back occasionally
Note : No tests recently done. Is it required to have a whole body checked up once ? Your advice will be really helpful

Overall could be acute exacerbation of rheumatoid disease.
Detailed Answer:
Hi Bhavik, I am Dr Gopal Goel , Orthopedic Surgeon.
I have read the details you mentioned in your complaints.
I would like to answer as follows
1. Shoulder- This is likely to be a frozen shoulder. Treatment is pain killers , physiotherapy and local injection , if required.
2. Finger and Wrists_ Swelling in fingers and wrists means increase in Rheumatoid activity. Get R A Factor and Anti- CCP antibodies . It may require addition of more and newer anti rheumatic drugs like Iguratimod.
3. Knee Pain - Again could be due to increased rheumatoid activity or Severe arthritis . Go for Local Injection.
4. - Back Pain : may be due to muscle weakness or Osteoporosis . Go For Vit D3, Se Calcium, And Bone Densitometry , X Ray L S Spine.
