Is It Safe To Give Mox 250 For Fever To A 3 Year Old?
Question: Hello Doctor,
My 3 Year old Daughter has started getting fever from last night. We gave Meftal Syrup and it reduced. But again started in the morning. Her Pediatrician has given some medicines & I just would appreciate a second opinion.
We live in XXXXXXX XXXXXXX where many kids are getting viral fever in last a few days. Doctor has recommended to use Meftal for 3 days and despite that if fever persists then has asked to for antibiotic: Mox 250. Could you please advise if you would agree with the medicine?

Hi XXXXXXX ..Fever of one day without any localizing signs could as well a viral illness.
Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require antibiotics at all. Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.
Paracetamol can be given in the dose of 15mg/kg/dose (Maximum ceiling dose 500mg per dose) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.
Meftal-P contains Mefenamic acid which is a NSAID. It carries the risk of gastritis and renal problems in the long run. Please do not use Meftal-P.
I am herewith providing a link to one of my publications in a reputed Pediatric Journal. Kindly g through it and you will be even more convinced about not to use Meftal-P.
I do not recommend using antibiotics either for a fever of one day and if at all antibiotics have to be started, they have to be started only after a blood culture.

Thanks indeed for your prompt and detailed reply. In the night temperature went upto 102. Now the fever has subsided and she is playing again (thankfully) although not with usual vigor. We will stop giving Meftal. (thanks). Our doctor had given another optional medicine to Meftal as T98 DS syrup. Appears that it is Paracetamol based medicine. We'll give her instead of Meftal. As the fever goes up, she complains of ear pain (she also has cough (sparse)). Doctor has given Drop wax ear drops. Please advise if that sounds ok. Doctor, as a final query, her appetite has reduced due to fever. In the night we give her 200 ml Milk and in the morning 200 ml Sofit (Vanilla). Can we increase her Sofit (for next a few days) as she is preferring liquid diet. Thanks again.....

Coming to your queries -
1. Ear pain as the fever goes up is quite common in viral illnesses due to Eustachian catarrh. But it could as well be an initial manifestation of otitis media or infection of the middle ear. I am sorry that I have to advise you against your regular paediatrician's prescription again - DO NOT USE ANY EAR DROPS.
What has to be done is - physician need to see the ear drum using an otoscope which is a very simple apparatus with light at one end to be inserted into the ear and if there is effusion or fluid in the middle ear or the drum is red, we need to start on antibiotics.
2. Appetite will be during fever...so do not worry about it. You can increase her Sofit or liquid diet, (for next a few days) as she is preferring liquid diet.
