Brief Answer:
Both the medicines are similar in action and are well tolerated

Detailed Answer:
Thanks for asking on HealthcareMagic.

Dibizide M contains glipizide 5 mg and metformin 500 mg. On the other hand, Gemmer forte 2 contains Glimepiride 2mg and metformin 500 mg. Both the tablets are similar in action and your doctor must have found some justification for the change based on the blood sugar levels. There should be no problems if this change is made.

Both of the medicines come among widely prescribed and well tolerated ones and I do not think that you need to worry about the side effects.

Hope that helps.
