Question : I need desperate help and support as my anxiety is killing me. About 2 years ago in 2015 I had some encounter as below:
7/dec:
I am male. Had unprotected brief blow job and protected vaginal intercouse with a female escort. Condom was intact and I did not notice any holes or tears. The only concern was she used her mouth to roll it down my penis after blow job for vaginal sex.
9/dec:
2 days later, Had protected vaginal sex with two different female escorts. Also had protected oral. Condom used was durex. Different condom for each escort. Condom did not appear to break. It was used end to end. Only note here is that the escort used same condom they did oral and did not replace with a new one for vaginall intercouse.
14/dec:
Did a blood test using cmia ag/ab method for HIV 1 and 2. Result was non reactive. This was 4.5 days post exposure.
23/dec:
Did another blood test with following results. This is 14 days post exposure:
Elisa ag/ab test for HIV 1 and 2. Result was non reactive.
Chlamydia and gonorea was also negative
Herpes 1 and 2 was also negative
Syphilis was also negative.
To-date I have no symptoms apart from fear and anxiety. Of course 2.5 yrs have passed with general cough or cold and fever and sore throat, but am seriously depressed and fearing now.
Last week had a lipid profile and cbc. My wbc count was normal but lymphocyte count was 1.4 (range 1.5 to 1.4)
Question is am I safe from Hiv considering the above? Do I need to still test? I have not had any relationship ever since 2015 and am in fear for even sex with my wife.
Please do review and help me doctor.
Thanks.

Brief Answer:
Please don't worry! You are free of HIV.
Detailed Answer:
Hello,
I have read carefully your history, having protected sexual intercourse (even with the same condom used for oral) and blowjobs I think you shouldn't worry because there is no risk for HIV.
Furthermore , having a negative result of 4th generation HIV test and STD after 14 days of exposure and normal value of WBC and lymphocyte I think is a good news and there is no need for further test
So, definitly I think you are free of HIV and you should continue to live and have sexual intercourse with your wife without problem.
I hope my answer helps you.
Regards.

Thank you doctor for your feedback. The issue is my hiv test was done 14 days after exposure in 2015. I read that this is not accurate. Now it has been 2.5 years (2018) after the incident and my lymphocyte count is low by 1%. Reading on the net that
this is Hiv related. I never did any hiv test after thr last test which was 14 days after exposure. The lymphocyte was tested as part of general screening on 10 May 2018.
I don't recall any ars symptoms in the last 2.5 years expect for some mild sore throat and cold or cough with fever around 38 degrees. Does symptoms really appear?
Do I need to test again since 14 days 2.5 yrs ago is actually not conclusive?

Hello Doctor, I have uploaded two reports
First report was when blood was taken on 14 Feb 2017. No hiv test taken. Only general cbc. This is roughly 1 year 2 months after exposure.
Second report was when blood was taken on 10 May 2018. No hiv test taken as well. This is roughly 2.5 yrs after exposure.
I also want to inform you that in 2017 I started some diet plan to lose weight and lost almost 7kg to date.
Again I want to highlight that I have no apparent symptoms apart from seasonal cough and cold with fever 2 or 3 times in 2.5 yrs. I was also travelling around.
Is there any risk? Considering my acts were all protected and condom did not fail?

You are free of HIV.
Detailed Answer:
Hello,
Based on your data(sexual history and lab results) I think you shouldn't worry because you are free of HIV.
I have seen your recent full blood count and I think that this value (mild lymphopenia) is not related to HIV.This could be caused even in viral situations (flu).
I suggest just to repeat the full blood count after 2 weeks. There is no need to do other HIV test.
I hope my answer helps you.
Regards.

Hi doctor, thanks for the feedback.
Is my 14 days post exposure test done 2.5 yrs ago accurate? I am reading alot about tests only being considered conclusive after 3 months.
Also, is it true that ARS symptoms never appears to most PEOPLE? I am concerned here because I did not get any, but did get fever and cold 2 to 3 times in the XXXXXXX 2.5 years. Fever was about 38 degrees and subsided after a day.
Ars symptoms if any would they have appeared by now since it is already past 2.5 years and would if hiv positive would surely show some signs of the disease no matter what during the last two years?
Am sorry for too many questions as am very stressed.

Brief Answer:
Please don't worry!
Detailed Answer:
Hello,
Yes , is it true that in some persons ARS symptoms never appear.
As well , according the recommandations after a risky sexual behaviour needs to do a 4 generation HIV test after 3 months from exposure as a conclusive test.
But, in your case , having protected sexual intercourse I think you shouldn't worry because there is no risk for HIV and is not necessary for other HIV test.
I hope my answer helps you.
Regards.
