Isolated big toe pain is unlikely to be related to the heart attack.
The presence of other symptoms like chest discomfort, Pain that Spreads to the Arm, pain that radiates down the left side of the body, dizziness, lightheadedness, getting exhausted easily, Nausea, Indigestion, Heartburn, or Stomach Pain, sweating, irregular heartbeat should require evaluation.
But if you are anxious it is better to get CVS examination done by physician.
