Is Painful, Burning And Swollen Ulcer Like Sore On Vulva A Sign Of Herpes?

Question: I have an ulcer like sore on my vulva at the vaginal opening. At first without looking at it and only feeling it with my fingers I thought it might be a small cyst because I sometimes get those on my ears and have had one on my vulva before but they are usually bumps under the skin that are tender to touch but go down after a few days. The next day I decided to look at it with a mirror and it seems to have morphed into a blister/ulcer looking thing that has sort of a white/yellowish head with darkening/blackening around it. At first it was only painful when touched but that night it became so painful when not even touched that I couldn't sleep, it was sort of a burning/stinging painful swelling sensation which makes me think it is infected. I put some anti fungal cream on (because I couldn't find newsprint or anything) and took some Motrin PM and the pain subsided so I went to sleep. There is no rash, no itching and it seems to be just the one lesion. I had very rough sex a little over 2 weeks before this happened and I was very sore in the vaginal area for around a week after that sex but I started my period that week and used a tampon a couple times even though it hurt to insert it. Is it possible this lesion is an infected cut/sore that hadn't yet healed after the sex? I am very very nervous about it being an std especially something incurable like herpes but from all the photos I've looked at of herpes none match what mine looks like, from my research herpes seems to look more like a red rash group of pimples or blisters which can itch and hurt and apparently takes a while to heal and crust over whereas mine is just the one spot on the vulva/vaginal opening which showed up roughly 48 hours ago and is already an open sore type thing. Also I don't know if this information is relevant but the day before I noticed the spot I contracted a stomach flu and was very nauseous and had diarrhea (which I'm worried could've contaminated the vagina and infected that spot) and I also used a sex toy that I hadn't cleaned so I'm worried that also could have infected the spot. I have an appointment to get std tested in a couple days to be sure but it's a holiday today and doctors are closed today and I just want some peace of mind because I've been sick with anxiety these past two days.

I just posted the photo. Seeing it up close like that makes me even more nervous. There seems to be two spots, one big and one small and it looks very infected. Please, tell me what you think!

Herpes ruled out for sure; friction wound is a possibility, advise given

Detailed Answer:
Hello once again ma'am.

In my opinion herpes is completely ruled out, and your lesions can quite possible be associated with friction wounds, but in all such cases I would always think it is best to get an STD-panel conducted just so we can be sure all the more concerning conditions can be ruled out.

But a friction wound can quite possibly present in such a manner which could have indeed been infected from the unclean sex toy you used. My advise would be the following:

+Application of clindamycin vaginal cream over the region twice daily for 14 days
+Avoid masturbation and the use of the sex toy
+Avoid sexual contact in all possible forms until at least 2 weeks after the completion of your treatment
+Rough sex is fun, but also unsafe at times so please do try to be less rough (at least in comparison to the last time)
+Let me know if you can get your hands on oral metronidazole tablets and if you can, then purchase 14 400 mg tablets and let me know
+Let us wait for about a week after initiation of this course to see if you respond well enough, and if you do not then it would be best to visit your doctor for the STD-panel and appropriate management; but if it is indeed just an infected friction wound, you should recover soon enough
+In the future, also make sure you are well lubricated (Either by indulging in adequate foreplay before penetration or by using safe artificial lubricants like KY jelly)
