Question: Hi mam,
I had my previous periods on 6th may and I had sex on 20-21 may . I took unwanted 72 after four hours due to condom failure. Now its 10th may ,my periods still not coming. I took home pregnacy test in morning using iCan but that showed one fainted line and one dark line. Am I pregnant ? What should I do to avoid pregnancy

1) I could assume that your last period date is 6th April and did sex on 20-21 April. It is not May as per your given data. I have corrected it.

2) If your previous cycle was quite regular (28 to 30 days duration), then you did sex close to your fertile period or ovulation day and chance of pregnancy was there.

However, you took unwanted 72 pill within 4 hrs and it will give more than 95% protection against unwanted pregnancy. In this case, pregnancy should be confirmed.

3) Faint line on pregnancy test kit is always doubtful. I suggest you to repeat the pregnancy test or undergo blood test for beta-hCG to confirm pregnancy. On reviewing test result, next management can be done.

Hello Dear,

Welcome back again. Followings are my comments:

1) On POSITIVE test, medical abortion should be done UNDER supervision of local gynecologist (need proper direction and complete prescription) and it will help in emergency situation. It can be done up to 9 weeks in 95% cases.

There will be some adverse effect like excessive bleeding, abdominal cramp, vomiting, fainting etc which CAN'T be controlled at home WITHOUT doctor's supervision.

2) Following is the general guideline for medical abortion:

a) Do one Trans Vaginal ultrasound (TVS scan) to confirm gestational age and nature of pregnancy (uterine or tubal) before taking the abortion pill. Tubal pregnancy is more dangerous and it needs different treatment.

b) Ideally, 1 Mifepristone tab is taken in the morning, which is followed 24 to 48 hrs later by four Misoprostol tabs (2 orally and other 2 vaginally). These are prescription only medicines.

Immediately after taking those pills, you will get vaginal bleeding and strong abdominal cramp till fleshy mass or clots are expelled out.

c) You may get bleeding or small clots for 1-2 weeks to resolve the abortion completely. Next, follow up examination is required after 1 week to check for retained bits or placental tissue. One pelvic ultrasound (post abortion) is quite helpful for confirmation.

d) In failed medical abortion (5% cases), surgical procedure (D & C) is required to evacuate uterine cavity completely. In such case, gynecologist can help you.
