Is Pregnancy Possible Through Non-penetrative Intercourse?

Question: Hi doctor I had gone to a massage parlour last week. She did a body to body massage and my penis was erect. This happened for less than 10 minutes till I ejaculated. During this time my penis rubbed her vagina a few times and I am NOT sure if her vagina was wet (to best of understanding it was'nt as I didnot feel any wetness). I did not have ANY sex - penetrative sex or oral sex and also the body to body touch was for less than 10 mins. Since that time I am under tremendous stress thinking / fearing if she would have ejaculated too and JUST IN CASE her vaginal fluid would have touched my penis. I am scared that is it possible her vaginal fluid would have taken the way through my penis urethra ?? Can HIV spread like this. I am extremely scared and can not concetrate on any work. I am married and since then I am avoiding any physical contact with my wife too. Please help in giving detailed answer. I would really be thankful for your help

There is very minimal chance of transmission of HIV by the modes described. It is less than a normal vaginal intercourse. The chances of transmission of HIV by unprotected vaginal intercourse with HIV positive person is 1/200 to 1/1000.If she is negative there is no chance of transmission.
You can go fo HIV tests based on time since exposure.You can get her tested too or ask for HIV status. You can go for HIV 1&2 RNA PCR at this stage.

Doctor I am sorry but your reply has made me more scared. I have been reading a lot of material on net and every where it said that body to body massage has NO risk even if the vaginal fluid touches the penis head. As I told you there was no vaginal intercourse and also NO cuts on penis or any blood from her side too.

I am quiet sure that she was not wet but just in case if it was does even a touch of small quantity of vaginal fluid over the penis tip can caus HIV?? Pls elaborate as three days back I have got a bad throat / cold and fatigue.

My all routines have gone for a toss as I am scared. Also I read that for tests one has to wait for 3 months. Its a very long period to wait in such distress.

Please help in giving detailed explanation.

I think there was misinterpretation of the fact.I have said there is very minimal or very low risk.We cannot totally rule out once the secretions have come in contact.Further as you have mentioned there are no cuts on penis or any blood contact ,then there is almost nil chance of transmission.

Further ,If no secretions have come on contact there is no chance.Simple body massage will not transmit HIV.
The symptoms are non specific and can be due to simple respiratory infection due to respiratory contact with any person.
You can go for HIV 1&2 RNA PCR at this stage to relieve your anxiety.You can get her tested too or ask for HIV status, so that you can be totally relieved of anxiety at this stage.
Yes,Antibodies tests are conclusive after 3 months exposure.But HIV PCR tests are quite sensitive after 10 days post exposure.

There is nothing to panic.I am suggesting tests to relieve your anxiety.The chances of HIV is almost nil.

Dear Doctor

Firstly thanks for a detailed reply and being so patient.

After your reply I had called Dr Lal Path lab to enquire about the test as me tiles by you. They conduct HIV RNA Continuity PCR Test which cost aprox Rs. 4,700. I wanted to briefly ask you few things.

1> is it the same test as guided by you. If not please suggest any clinic in XXXXXXX where these test can be performed.

2> is this test conclusive and no antigen test is required after 3 months again

3> Can I trouble you once again on Saturday as the reports shall be available on Saturday once I give them the sample tomorrow.

4> Lastly I am sorry to ask this again but is it possible for very very little vaginal fluid to travel through the urethra if at all it touched the penis.

Sorry doctor I am bothering you again and again but trust me I am a lot stresses and your XXXXXXX are clearing my mind a lot. Appreciate your help and time a lot

The chances of HIV transmission as said earlier is almost nil.So you can allay fears. But Sometimes it is better to go for tests to allay fears.

I think it is HIV RNA quantitative test done at lal path lab. If both HIV 1&2 RNA PCR is done it is fine.Otherwise also it is okay.
You can go for this test just to relieve your anxiety at this stage.It is best test at this stage(>99%) sensitive.
You need not go for any HIV test till 3 months.You can go for antibodies tests to have final conclusive results after 3 months. Though the chances of positive results later is nil,it is better to have conclusive results.
You can get back anytime ,even on Saturday.If it is of help to you ,we will be pleased .
Just relax.

Thanks for the explanation and replying back all my XXXXXXX

I have given my blood samples for HIV-1 RNA quantitative real time pcr test this morning and i guess the reports would be available on Saturday. I shall mail you once again on getting the reports.

I am sorry for troubling you with lots of questions. I had cough and little pain in my throat last Thursday. I took Azithromycin 500Mg IP once a day for 5 days till it got much better but since I also had cold and little feverish feeling i started having anti allergic Tablet -,Cheston Cold (2 times a day ) and Benadryl (3 times a day) syrup from last TWO days. My cough and cold is much better and quiet ok but i still feel feverish and little sick with some cold although there is no fever as such.

I am really getting worried that why is it taking so long to get completely cured since it would be seven days tomorrow and still feel feverish with cold n mild cough.

1> Are symptoms of HIV also same.
2> I have a hectic schedule at work and get to sleep only for 6 hours - can this be a reason of prolonged cold / feverish.

Doctor i am really tensed please help me with your explanation on above. You are doing a great help.

Hi Doctor

Thanks for all the support you have been giving me in last few days.

I have got the results of Lalpathlabs just a while back for HIV-1 RNA Quantitative @ Real Time PCR Test. The report shows "Target Not Detected" and the interpretation mentions - Sample provided does not contain HIV-1 RNA.

I don't know actually how to react and should I be at peace or still live with fear. All these days have been really a hell. Just to recall my exposure was limited to body to body touch and my penis would have rubbed outside the masseur's vagina. There was no vaginal intercourse / oral sex or deep kissing. But I fear of at all there was a vaginal discharge and if at all it would have accidentally touched the tip of penis tip.

Doctor I need your help to answer the following questions with your advice and good karma for me to be clear in my thoughts.

1> Are the results of RNA PCR Test reliable.

2> After the results of RNA PCR continuity test should I still go for Antigen Test after 3 Months. Does that mean I live with the fear for all these 3 months.

3> I have been avoiding any physical contact with my wife in all these days. Should I not involve and abstain in any deep kissing and protective sex also with her.

4> My cough and cold are quiet ok but not completely gone. Hope it has nothing to do with HIV.

Doctor I know I have been troubling you with some repetitive questions but I am really scared and carrying an abnormal behaviour.

Please help with detailed reply.

As i have said earlier the risk is almost nil.

RNA PCR is a very sensitive test at this stage(>99%).It has come negative .So relax at this stage.You need not go for any HIV test till 3 months.You can go for antibodies tests to have final conclusive results after 3 months.

Though the chances of positive results later is nil,it is better to have conclusive results to allay your fears totally.
You can go ahead with protected intercourse with your wife.You can discuss with your wife if you desire to.
