Question : I am 28 years old and my girl friend is 29 years old and she is a virgin. We had oral sex on 3rd, August 2017 at 9:30 pm in the night. I was in trouser and she was in trouser and also wears underwear at that time.

I rubbed my penis on her vagina for 1-2 minutes. There was some precum/pre-ejaculated fluids wet area on my trouser.

I did not penetrate my penis into her vagina did not rub nakedly. I just rub my penis on her vagina and there are 3 layers of cloth in between penis and vagina. 1- My trouser(pre cum wet area is there), 2- her trouser and then 3- her underwear.

I also rub her vagina with my hand from the top of clothes. I did not feel any liquid on my hand. And she said that my underwear is absorbable that's his vagina water is not coming out.

I ejaculated on her belly/stomach which we wipe off with tissues. Then after 30 minutes, she cleans only her belly/stomach with soap + water without removing her trouser and underwear. Then after another 30 minutes, she again first wipes off her belly/stomach with water + soap without removing her trouser and underwear. Then She took bath and changed all her clothes. But we both are worried about pregnancy even we can't sleep the whole night.

And Then on next day i.e. on 4th August 2017 at 2:00 pm afternoon(after 16 hours) she took ECP (Levonorgestrel 0.75mg) 2 tablets as per the recommendation of the pharmacist. She was on her periods on 19th July 2017(First day).

We do this activity on 16th day's night after the first day of periods.

We are worried and have following questions.

1- What are the chances of pregnancy in that way without penetration only from pre-cum and 3 layers of clothes between the penis and vagina? Remember we did not penetrate not rubbed our genitals directly/naked just by rubbing genitals and layers of clothes are in between us

2- How is ECP is beneficial in that situation? we did not know that whether she is ovulated/fertile or not. It was her 16th day of the cycle from the first day and the last cycle was 33 days long.

3- Is that was the right decision to take ECP in that situation. She took ECP (Levonorgestrel 0.75mg) 2 tablets simultaneously after 16 hours.

4- Is ECP affected on mensuration cycle of all women or only for some?

We read many threads on the internet and become more confused after that I think it will be the best to ask a question.

Please help us and sorry for long detail Waiting for your reply :(

Hello and welcome,

Here are my answers to your questions:

1- What are the chances of pregnancy in that way without penetration only from pre-cum and 3 layers of clothes between the penis and vagina? Remember we did not penetrate not rubbed our genitals directly/naked just by rubbing genitals and layers of clothes are in between us

There is no chance of pregnancy from this activity. There has to be semen with viable sperm on or in the vagina to cause pregnancy. Sperm cannot penetrate clothing.

2- How is ECP is beneficial in that situation? we did not know that whether she is ovulated/fertile or not. It was her 16th day of the cycle from the first day and the last cycle was 33 days long.

ECP is not necessary in this situation.

Hormonal ECP is a high dose of progestin (like progesterone which is the "hormone of pregnancy") which thickens cervical mucus and makes ovulation and implantation less likely. Unfortunately, a high dose of progestin can also throw off her normal cycle, so please don't worry if her period is late or early or less flow, or heavier.

The least fertile time is immediately before or after the period. If the 1st day of the month is defined as the 1st day of the period, around day 14 (if her cycles are 28 days) is the most likely time for ovulation (give or take a few days).

3- Is that was the right decision to take ECP in that situation. She took ECP (Levonorgestrel 0.75mg) 2 tablets simultaneously after 16 hours.

I don't think it was necessary, but it is unlikely to cause harm just used this one time.

4- Is ECP affected on mensuration cycle of all women or only for some?

Taken within the first 72 hours after unprotected intercourse, it is fairly effective, but not as effective as regular birth control, such as The Pill. It also should not be used frequently - just in an emergency. Certain brands of hormonal ECP are not effective in women over 165 lbs.

I hope this helps. Please let me know if I can provide further information.

Thanks, Dr. For you detailed answer. I just want some more clarification.

According to your answer,

1- Pre-Cum can not cause pregnancy. There should be semen with viable sperms on and actual penetration is needed because the vagina is inside the body.

2- Sperms cannot pass through or penetrate in clothes. So the wet area or some amount of precum from our clothes does not contain sperms.

3- What will be the best diet for her for next few days which will increase her chances to get periods on time.

4- We do not need to worry anymore. Because there are no chances of pregnancy in our case.

Detailed Answer:
1. Pre-ejaculatory fluid either has no viable sperm, or only a few sperm. So the chances of pregnancy from pre-cum are extremely low to non existent.

Depositing semen in or around the vagina can cause pregnancy. It does not actually have to be inside the vagina (although that increases the chances). There are cases of pregnancy where semen was on the outside of the woman's genitals (but at some point entered the vagina). So if semen is on the outside of the vagina (without clothing on), that can cause pregnancy too.

2. Sperm require fluid to travel (and to remain alive). So they wouldn't be able to travel across your clothing easily.

3. I do not think that her diet will effect her cycle reliably enough to ensure that she gets her period on time if she has had a high dose of progestin. But eating healthy foods and avoiding junk food is generally a good idea. And avoiding stress if possible.

4. No, there is no chance of pregnancy from the situation you described where there was 3 layers of clothing and no actual ejaculate. So no worries. If her period comes late - still no worries.

5. In the future, consider using a condom with spermicide instead of ECP might be a better option.
