Question: Hello Doctor,

I had unprotected sex with my wife but pulled out before ejaculation. We had sex again after 8 hours and used the same pullout technique before ejaculation. However, i am worried of an accidental pregnancy from the leftover sperm in the urethra from the ejaculation 8 hours earlier entering the vagina with the pre-ejaculate liquid. Should I be worried as I did not urinate in between? How long do sperms survive in a male urethra?

Sperms are passed through urethra along with seminal fluid If you try to pull out just before ejaculation, it may not be perfect but some may be deposited in vagina, leading to pregnancy. so there can be pregnancy-yes. but not due to sperms left behind for 8 hours but those deposited due to incomplete pull out . No wonder, it is a very poor method of family planning. You should get a urine pregnancy test done if she misses her period.

I am certain of having pulled out completely and am not worried of having ejaculated inside her vagina. My concern is only the leftover sperms in urethra entering the vagina. What are the chances? How long do the sperms survive in the urethra? Is 8 hours enough to kill them inside the urethra?

What additionally can be done as a follow up measure? Do you suggest my wife should take a contraceptive pill considering that this happened just yesterday?

I was referring to the majority of people and scientific view. If you are very very sure, it is good. The life of sperm in urethra is till it passes with seminal fluid. The sperm is nourished only with this fluid. Since the seminal fluid is not there is urethra sperms can not survive on their own in urethra so I don't think you need to worry about pregnancy. The seminal fluid reaches the urethra again during next orgasm.
Just wait and watch, if she misses period for 3 days, test for pregnancy in urine. In my view, no pill is needed.

So, as I understand from you, the leftover sperm in the urethra from the last ejaculation is without the seminal fluid? Does that mean that the leftover seminal fluid oozes out completely in the period between the two ejaculations? If I understand correctly, what you are referring to as seminal fluid is different from the pre-ejaculate. Please let me know if that is correct.

I just wanted to know this for my reference and to be completely sure of the situation.

No, there are no left over sperms from previous ejaculate. If a few hundred sperms are left behind after ejaculation, they can't survive in the environment because urine is acidic in nature . This is the reason the cowpers glands and paraurethral glands need to secrete a sticky fluid before sex . This is commonly mistakenly called semen by many and precum as correctly. Possibly what you see after ejaculate is secretions from prostate which coagulate and pushed out with urine Seminal fluid is from seminal vesicles and prostate where as preejacculate is pre cum as explained above.
As far as seminal fluid from previous ejaculate is concerned, it is a complete misconception and any seminal ejaculate does not stay in urethra for more than a couple of seconds so no chance of previous ejaculate in the ejaculate 8 hours after. the seminal fluid from one ejaculate comes out completely , is any sperms are left, they are destroyed
