Is Pregnancy Possible While Missing Contraceptive Dosages?

Question: I started a new pill pack on October 4th (I think I started a few hours late, but less than 24 hours). Had sex on 9th night (6th day). Now I forgot to take a pill on 13th (10th day) and had sex that evening. I took the pill on early morning on 14th (9 hours late from when I was supposed to take it). 1) Should I use emergency contraception pill now? 2) What should I do next? Is extra protection needed further?

Dear doctor, I think I messed up again (looks I'm so unlucky these days...).
After missing a pill on 13th October I continued taking the pills regularly and did not have sex for 5 days. I had sex first today on 18th October with a protection as you advised (condom).
Unfortunately the condom slipped off and I'm not sure if something could have happened. What should I do now? Should I take an emergency pill now? Or am I safe at this point (it's the beginning of my 3rd week of pills now and I expect my period on October 25th).
