Is Pulse Rate Of 45 Bpm In An Elderly Person A Cause For Concern?

Question: My pulse is 45 bpm, age 62 female, with heartrac ecat. Are bpm's in the 40s going to require a pacemaker? Will have nuclear stress test after monitoring is complete. Thank you.

First, I would like to explain that a heart rate of 45 bpm may be just a normal sinus rhythm with bradycardia, but also any atrio-ventricular conductance disturbances (second or third degree atrio-ventricular block).

(1) If atrio-ventricular block is confirmed by continuous cardiac monitoring, then a clear indication of pacemaker implantation would be the rationale choice.

(2) If only sinus bradycardia is present without any significant symptomatology (profound weakness, dizziness, presyncope or syncope), then a close and periodic medical supervision without deciding immediately for pacemaker implantation would be necessary to exclude a hidden sick sinus syndrome.
