default
Posted on Wed, 19 Aug 2015
Question: Hi, ive written to you before about my moles. Im so worried. In the last year i have got five moderately aytpical moles diagnosed. They were tiny. On monday i will have number five excised. In the meantime i found one that i think changed. I have sent you two pictures one of now and one of the mole from a year ago. Could it be a sebbhoric keratosis? Or bad mole? Im scared. The first one is now the second photo is from mole mapping

Not melanoma

Hello
Thanks for the photograph.
Melanoma presents with the following symptoms.
1. Sudden increase in size.
2. Pain and tenderness in the moles.
3. Appearance of 100 s of moles suddenly.
4. Bleeding in the mole.
5. Systemic symptoms like fever weakness etc.
Just relax it's just a normal mole.
If you are too much worried get a biopsy done.
Let me know if you have any other doubt.

I notice that it looks a little assymetrical and reddish. With my other aytipical moles i had the same thing. Do you see different shades of color? Etc

Answered by Dr. Dr. Rahul Kumar (18 hours later)
Brief Answer:
Not melanoma

Detailed Answer:
Hello
I agree that there is danger of development of melanoma in atypical moles but the risk is very low.
I saw that there is variation in color but only that cannot be taken as a sign of melanoma.
At present you just have atypical moles.
There is no evidence suggestive of melanoma.
Just relax.
Let me know if you have any other doubt.
Thank you

Hi,

Its so stressful i dont underatand why this is happening? Why im getting these diagnosis of modertely aytipicAl moles? And does this mean i will get melanoma or that i have a high risk of it? I am going again to the dermatologist tomorrow. I feel nervous and anxious. If you can calm me in anyway and help me understand this i will be most appreciative . Many many thanks

Answered by Dr. Dr. Rahul Kumar (4 hours later)
Brief Answer:
Not melanoma

Detailed Answer:
Hello
Moderately atypical means that all the cells are not similar.
It doesn't mean that you are going to develop melanoma.
In melanoma the melanocytes start invading other tissue.
No such thing has happened to you in past years.
I have seen cases of atypical moles who were diagnosed more than 20 years ago and are totally healthy at present.
Just relax atypical mole doesn't mean melanoma.
It's a regular diagnosis.
Better get all the moles excised.
Let me know if you have any other doubt .
Thank you

Hi.

So I had two biopsies the one you saw was fine but I had a new strange one in my leg. It was a tiny mole that was flat last year! I tried to send you a photo but I couldn't on this re answer site. I will try next time. So it became dark brown and raised, literally overnight ! It turned out to be an actinic keratosis. This is my second one. I feel really sad. It's been two years since all my aytipical moles started. Now I've had five small ones and two actinic keratosis appear. I'm 38 and this started two years ago. I feel such anxiety. It's been years that I'm hardly in the sun. I protect myself all the time. In my teenage years I took a lot of burns. Do you think my skin will calm down ? I know you mentioned you had seen this type of aytipa before but have you seen people have all this and not develop cancer. Asking for advice and calmness! Thanks
doctor
Answered by Dr. Dr. Rahul Kumar (17 hours later)
Brief Answer:
Nothing to worry

Detailed Answer:
Hello
Thanks for the follow up.
Actinic keratosis is a pre cancerous condition and must be related to your previous history of sun exposure.
There are chances of development of squamous cell carcinoma in these moles but the good thing is that there is no risk of melanoma.
Melanoma is a dangerous and unpredictable disease.
You don't need to worry at all just get these moles removed.
As soon as you see that a mole is changing it's shape or color get it removed.
Just calm down it is not going to involve your systems.
Let me know if you have any other doubt.
Thank you

Thanks. I think they called it a pigmented actinic keratosis. When in your experience does all this sun damage come out? Will my moles stop changing ? And does this put my risk of getting a melanoma higher? So far moderately aytpical moles and two actinic keratoses. Many thanks
doctor
Answered by Dr. Dr. Rahul Kumar (22 hours later)
Brief Answer:
No risk of melanoma

Detailed Answer:
Hello
Thanks for the follow up.
Pigmented actinic keratosis is a variant of actinic keratotis.
The potential to develop squamous cell carcinoma is there but there is no risk of melanoma.
Usually the sun damage is evident after 4 th decade of life and the risk remains same after that.
Just relax and keep a watch on your moles.
You don't need to inspect them daily.
Once in 15 days you can just have a look at your body thoroughly.
Let me know if you have any other doubts.
Thank you.

Brief Answer:
It's a normal mole

Detailed Answer:
Hello
Sorry for replying late.
It's looking like a simple epidermal mole.
Don't get scared.
Just keep a watch for the following.
1. Sudden increase in size when mole increases and become more than 1 cm in diameter.
2. Pain in the mole.
3. Bleeding in the mole.
4. Thickness of skin around mole.
If these things are not there you don't need to worry.
Let me know if you have any other doubt.

Please relax

Detailed Answer:
Hello
Thanks for the follow up and photograph.
Just with change in appearance it cannot be said that its going for keratosis.
Even if it turns out to be keratosis there is nothing to worry.
It can be easily removed.
Keratosis has chances of development of carcinoma but even that is rare.
All keratosis don't develop carcinoma.
Just relax there is no hurry when you find a doctor get it removed and sent for biopsy.
Calm down you are safe.
Let me know if you have any other doubts.

Hi youre so helpful thank you. Please tell me what you think. It is the same mole from before. This was a flat sun spot mole and then it became red and inflamed. Now a crust formed.. Do you think this is squamous or actinic keratosis? I feel so worried. I dont underatand why all this is happening. XXXXX

Answered by Dr. Dr. Rahul Kumar (16 hours later)
Brief Answer:
Actinic keratosis

Detailed Answer:
Hello
Thanks for the photograph.
It is looking like actinic keratosis.
Crusting is a feature of actinic keratosis.
There is nothing to worry about this.
Actinic keratosis may develop into squamous cell carcinoma but this happens only when it is left untreated for years.
Just meet your doctor whenever possible and get it removed and biopsies.
Let me know if you have any other doubt.
Thank you

Thank you. I guess my constant fear is what does this all mean? The fact that my skin at age 38 is coming out with a few actinic keratosis and as you know some moderate aytpia moles. My question is am i at a higher risk now of getting any forms of skin cancer? Last night the crust fell off! So strange. Can this happen also? Thank you !
doctor
Answered by Dr. Dr. Rahul Kumar (16 hours later)
Brief Answer:
You are safe

Detailed Answer:
Hello
You don't need to fear.
Actinic keratosis is not a very serious thing.
I understand your fear but rest assured you won't develop any serious disease.
Just keep a watch on the moles and get them removed if they look changed.
Crusting and falling of crust happens.
Just see that there should not be any ulcer below the crust.
Let me know if you have any other doubt.
Thank you

Sorry for repeating the same questions i guess i am so nervous right now. If someone like me has a few ak s and some moderate aytipia does it put me at a high risk of getting skin cancers /melanoma etc.?
doctor
Answered by Dr. Dr. Rahul Kumar (19 hours later)
Brief Answer:
No risk of melanoma

Detailed Answer:
Hello
Actinic keratosis is a premalignant lesion in which squamous cell carcinoma develops.
This means if actinic keratosis is left untreated for years it might develop squamous cell carcinoma in that lesion and not anywhere else on body.
There is no risk of melanoma.
I can understand your anxiety.
You don't need to feel sorry you can ask as many questions you want.
We are here to help you.
Let me know if you have any other doubts.

Thanks doctor,

I guess what i mean doctor is in the past few years i have had five smalll moderately aytipical moles (moderate) and these actinic keratosis. How high os my risk now of getting some bad skin cancer? And do you think the cause is sun damage? Im so scared.XXXXX
doctor
Answered by Dr. Dr. Rahul Kumar (18 hours later)
Brief Answer:
You won't develop cancer

Detailed Answer:
Hello XXXXXXX
I understand your concern.
Sun damage is definitely a factor.
As I mentioned earlier Actinic keratosis can develop into squamous cell carcinoma if left untreated for a long time.
This can be easily monitored, whenever you see a suspicious mole get it removed.
There is no risk that suddenly some malignancy will appear on your skin.
Just relax.
Feel free to ask if you have any other doubts.
Thank you
