Is Rapid HIV Test Done 28 Days After Exposure Conclusive?

Question: Hello Doctor,
I am 39 years old had sex with a female sex worker on 04-April-2015 night.
I had protected sex but 3/4 of my penis is covered by the condom and just before 2 hours going to the sex worker I shaved my unwanted hairs which was near groin area near penis and which shaving there were several minar cuts.
I had sex with the sex worker for 3-4 minutes and I was down and she was top of me and later on after 3-4 minutes my sperms got ejaculated in the condom and lateron her vaginal fluid which was water like was in my groin area and it has contacted the minor cuts which was in the groin area.
Immediately after that act she asked me to wash the area with the water and I have done the same and applied liquid soap as well. I dont know if she is HIV + or - or anyother STD things.
After 2 days I was having itching and rashes in my fingers and some part of the body like chest hands and my mouth was full of yeast and my finger nails have thin black color lines which is itching and there were night sweats completely drenttched the bed for 2 concecutive nights. After 28 days I got tested for RAPID HIV and SYPHLIS and was NEGATIVE,
After that there were lots of muscle twitching in my body and lots of twingling in my palm and also in my feet.
I was bearing and did not go to any Doctor and then on 58 day I went for RAPID test for HIV and SYPHLIS and both were NEGATIVE.
The Symptome were still persisting in my body and later on my finger nails tip was having itch and blood oozing out of that nails and my body color as well as my face color was dull and becoming dark and all my skin shine all have gonbe and my lymph nodes were coming out.
After 93 day I had tested for HIV and SYPHLIS and again the test result was NEGATIVE. But still the symptoms are there and there wer are bumper rashes inmy body but it was coming and going.
After that I was not having confidence on the RAPID test and after 113 days I had visited abother Doctor and tested all STD and STI diseases and all came NEGATIVE.
ALl the tests were PCR, ELLISA, WesterBlot and all came NEGATIVE.
Still there are symptomss continuing.
I had again all STD diseases test after 5.5 months and all came negative.
But still the symptoms are going on.
Later on 6 months I had all testes all came NEGATIVE.
After that one of the Doctor suggested for SWAB test after 6.5 month and later on after 6 days the result were saying that I was infected by the 3 Urinary infections 1. Reichlich prevotella bivia, 2. Enterococcus faecalis and 3. Vereinzelt Proteus Mirabilis. After 6.5 months my skin becoming more darker that usual and my chubby cheeks gone away and became very thin and my hip siye has becaome loose and whole body got loose previously it was fit as fiddle.
The Doctors gave me Co-Amoxicillin 875 mg/125mg twice a day for 6 days.
I still have the symptoms and I have lost weight and the symptoms are still present.

Question 1 - Are these NEGATIVE test results are conclusive.
Question 2 - Or Do I have undetectable VIRUS load in my body so that the tests were not able to find out, ccould you please explain on the same if this can happen.
Question 3 - Can the 3 Urinary infections cause all the symptoms which I have.
Questiion 4 - I have my 10 years old son. can these be contagious by touching or by kissing or by offering him food with my fingers. Also can I have sex with my wife withoout condom.

I also face burnig sensation and weight loss.

Please advise.

Hello. Thank you for writing to us

I have gone through your query and I have noted down your concern.

Your tests are conclusive and you are clear of HIV and STDs.

A rapid HIV test is conclusive at 3 or more months after possible infection and after that there is no possibility of seroconversion, as far as Hiv is concerned. Moreover, most of other STDs like syphilis and genital herpes would be detectable in blood test, if you would have got infected. Also since you did'nt had any history of urethral discharge, genital sores, warts etc therefore I don't think you acquired any of the infection which come under the category of STDs. Mind you, a UTI is not considered an STD.

Your skin problem seem to be different and not related to urinary tract infection.

None of the STDs would be transmitted by casual kissing or touching your son and since you are free of STDs therefore i don't see any problem if you have unprotected sex with your wife.

Hello Doctor,

Firstly thanks for your immediate response andd encouraging answer.

But the body which I had before 04-April-2015 is different and the body which I have after that is different.
Doctors ruled out the STD and STI diseases but what is the reason that I have these symptoms which is still continuing.
Why is my weight getting reduced and why am I loosing weight.
Why my body color and why my face color got dark color.
and could you please answer my previous second question.

I suggest a thorough list of investigations; no possibility of Hiv

Detailed Answer:
Hi.

The weight loss, enlarged lymph nodes and night sweats seem to be due to an entitely unrelated cause rather than due to UTI or related to that sexual encounter. A physician would be better able to help you with these general symptoms. I suggest a comprehensive list of tests including complete blood count, liver and kidney function test, X-ray chest and Fine needle aspiration cytology (FNAC) from one of the enlarged lymph nodes e.g Tuberculosis is one such condition which needs to be considered and thoroughly investigated. You have all the consetellation of signs and symptoms suggestive of Tuberculosis.

There is no possibility of an undetectable Hiv virus, specially with all the tests i.e ELISA, WESTERN BLOT, PCR as negative. One thing is for sure and that is you don't have Hiv.

Dear Doctor,
I am in Germany and the Doctors say that Tuberclosis will have heavy cough and fever. But i never had these symptoms.
My body is become very loose and complexion is become dark.
Please suggest.

Hi.

Tuberculosis is just one possibility that I illustrated as an example because of the constellation of common signs and symptoms of tuberculosis that seems to match with yours. Of course, firstly the diagnosis needs to be either confirmed or ruled out with appropriate investigations. Nevertheless, 'lymph node tuberculosis' does not present with cough and moreover, fever is low grade and associated with night sweats and other systemic symptoms like malaise, weight loss etc. Cough is more of a feature of 'pulmonary tuberculosis'.
The point I wanted to make is that you need to be investigated thoroughly for all the underlying causes that may give rise to these constellation of signs and symptoms and tuberculosis is just one of those causes.
