Question : HI doctor

I am 25 year old male had a question when I ejaculate say after 3 days the first ejaculate is always greater. Definitely a Good amount comes out say atleast 3/4 of a tea spoon just for example.

I notice when I then masterbate the 2nd or the 3Rd time the ejaculate decreases? Is this natural? The first ejaculate is always the greatest but 2nd or 3Rd it does decrease why is this? I don't really need a sperm count test as I am not married.

But it always is good to know why this happens, is it a natural process? I'm perfectly healthy.

What would you recommend for increased sperm? The most I have ever had a break for is 2 weeks it is a natural process right?

Dear patient,

what your are describing is completely normal and natural.

This is due to the natural sperm production cycle in the testis which needs a few days. There is no indication for a sperm count since there is no sign of anything unnormal. There is no safe or proven remedy for increasing the sperm volume. Medically spoken you also do not need to do anything.

Just one follow up question WHO suggests that sperm sample should be around 2ml.

Should it be 2ml's everytime upon ejaculate or is this not possible due to the sperm cycle?

Because obviously the first ejaculate will be the most and get rid of the volume at once? But other reserves may be less

So my final question is as long as the sample is a decent amount..after 3 days does it matter if the second or third is of low amount and does this have a affect on sperm count as when practising intercourse trying for a baby you can't rely on 1 load every days right? Or am I worrying about nothing.

the WHO 2ml information is an average and may vary from person to person and depends on ejaculation frequency. When trying for a baby after approx. 2-3 days ejaculation pause chances are mostly the highest.

Dear patient,

i can say that the ejaculation volume/ quantity is normal. However, if there is any doubt about your sperm quality (only in case you are trying to make a baby and it does not work) it would be safest to also make sure sperm quality is normal. This gets diagnosed by a spermiogramm.
