Brief Answer:
Please do not panic, it is not an acute emergency condition

Detailed Answer:
Hi XXXXXXX
Do not panic so much regarding Supratentorial microangiopathy.
It is the pathology involving the small branches of the blood vessels in the brain and it is not a sudden development or acute illness.
It develops over the time and there can be various causes for it like:
1. Hypertension
2. Dyslipidemia
3. Diabetes
4. vasculitic diseases, autoimmune diseases.
You should get your blood pressure checked, your blood sugars, Liver and kidney function tests, and lipid profile, vitamin B12, ANA profile and complement levels done to ascertain the cause and get appropriate treatment.
You can get back to me with your MRI images so that I can review and comment upon them.
Hope you found the answer helpful. Please do get back for further queries.
Wishing you good health.
