Question : do you have to have surgery to unclogged a tear duct? Or is there a less scary way to get it done?

It seems from the history that you are having blocked tear duct and it is confirmed by your CT scan test.
The treatment of this condition is by a surgery called as dacryocystorhinostomy.
This reestablishes the patency of the lacrimal drainage system.
This can be done by three methods.
1. Conventional one by skin approach: This leaves behind a scar on the skin.
2. Endonasal approach: This is usually done by ENT surgeon.
3. Laser surgery: This is done by laser introduced through the pun tum and canaliculi of the eye.

Hence if you want to have surgery without scar then you can choose either 2nd or 3rd one.
