Question: can you tell me if this swelling is water retention or general swelling? my ankle and foot have been painful for at least six months.

Hi,

Always a painful and unilateral swelling of an ankle and foot should be checked thoroughly with a blood test, color Doppler of the vessels, signs of local infection.

If it is painful it is most likely be inflammatory exudate and there would be fluid piling up in that part of the body. There could be pressure in the vessels bring the fluid called transudate. There would be generalized swelling in both the occasions.
Nevertheless, compression stocking has to be used all day, keeping the leg above the knee level should be taken care.
I suggest getting blood counts, ESR, liver enzymes, and color Doppler.
