Is Testosterone Replacement Therapy When Diagnosed With High-risk Prostate Cancer?

default
Posted on Sat, 1 Aug 2015
Question: Please read my history of high-risk prostate cancer in my profile. It's been one year since completing salvage radiation therapy and ADT. My PSA has been undetectable for one year. My T levels is at 243 (reference range of 211-716) I want to know if I am a good candidate for testosterone replacement therapy? My doctors say NO WAY. What's your professional opinion.

Brief Answer:
Recent research might support testosterone replacement therapy

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. I am in the knowledge of recent medical research debating the benefits and risks of testosterone replacement therapy in patients who are treated for cancer of prostate. This matter has been under a lot of controversy with different groups of doctors having varied opinions. Some of the controversy seems to have been sorted out with continuing medical research.

2. The recent research by a team of doctors from University of
North XXXXXXX in the year 2011 indicates that testosterone replacement therapy might be initiated in the group of patients who have been treated earlier for prostate cancer.

3. The researchers have specified that the gap between cure from prostate cancer and starting the testosterone replacement therapy should be at least 6 months.

4. In view of the above opinions, it would be important to know when you had the pelvic lymph nodes detected and the duration since last treatment.

5. Your testosterone level of 243 is at lower limit of normal and further decision on any testosterone treatment therapy is to be taken after discussing the risks and benefits in detail.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

You asked:
>>it would be important to know when you had the pelvic lymph nodes detected >>and the duration since last treatment.

My answer:
The doctors detected positive pelvic lymph nodes in 2013. I had radiation therapy in the winter of 2014, with ADT. Last ADT treatment was early April 2014.

Brief Answer:
Starting testosterone replacement therapy is an informed decision

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.

1. Since it has been over 6 months that you are free from the disease, you might be considered a good candidate for testosterone replacement therapy by some doctors.

2. The risk of recurrent cancer of prostate after testosterone replacement treatment as mentioned in several researches is in the range of 1 to 1.8 percent in patients who have been treated in the past for cancer prostate.

3. However, it is true that the controversy persists and some doctors would not take the slightest risk and find testosterone replacement therapy unsuitable for patients who are treated for cancer prostate.

Every recommendation of testosterone replacement therapy requires detailed discussion and understanding between the doctor and patient and considering the risk and benefit, treatment might be suggested.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
