Hi Doctor.
I had a dog bite and had a 5 full vaccination in 2012.

After that on Aug 20th i had another dog bite and took 2 booster injections.

Its been two weeks since then and yesterday morning i had dog licking in to my open wound which i did not notice and noticed later. It was a stray street dog.

1. Having previously vaccinated and with booster 14 days back, do i need to get booster injection again?

2. I use Rabipur injections. or the booster i had 2 weeks back is enough to protect me?

3. having another 2 injection will make overdose and cause side effects? or is it okay to go ahead?

Please help ASAP

Brief Answer:
May have to get Vaccinated again.

Detailed Answer:
HI
Thanks for posting your query to Healthcaremagic .

If you can watch the dog for 10 days and nothing happens then there is no need for any Vaccination .
If you can't watch the dog or if the dog dies in 10 days then you need to get Rabies vaccine again that is a two dose injection .

There is no problem in taking the Rabipur again , no issue of overdose or side effects . You can go ahead and get vaccinated .

Any clarifications or Queries feel free to ask.
Happy to help you.
Regards.

Did you consider the fcat that i had already received two doses of booster injection 2 weeks back?

Does the increase in level of antobody causes rabies encephatilis or any lymphoma kind of disease?

How long the booster vaccination will be effective?
doctor
Answered by Dr. Divakara (11 minutes later)
Brief Answer:
Yes I have considered.

Detailed Answer:
Hi

As per guidelines any reexposure after the last vaccination dose needs revaccination .
It is not like you take one course of vaccination and you are protected for certain time . There has been no reported cases of Lymphoma or encephalitis due to re vaccination. Yes, encephalitis has been reported during first course of vaccination. If you didn't have problems during initial vaccination subsequent vaccination with Rabipur will not cause any problem .

Any clarification feel free to ask .
Regards.

Hi Doctor,

In that case, with 100% assurance, can i go ahead with another one whcih is pending on Day 3 without having any fear of Lymphoma or encephatlies?

Please confirm for the last time.

Am i 100% out of danger with excess dosage of rabipur vaccination?

Brief Answer:
Can Guarantee 99%

Detailed Answer:
HI

See nobody in this world can give 100% guarantee for anything especially in the field of Medicine .

A person who is taking the same injection every month for 15 years developed a serious reaction to that same injection once . So side effects and adverse reactions is always a possibility with any medicines.
But since you have tolerated the Rabipur injection well I can safely say that it won't cause any problems if taken again. But 100% assurance I can't give .

Apologies for that.

Hi Doctor,

To provide my history, I had rabipur 5 injections in 2012 from July 2nd to July 30th. Then i had to have two booster shots on Aug 15th and 18th because
i was bitten again. My bad luck :)

Then now in 2014, i had two booster doses on 21st of Aug and 24th of August and all was fine.

But again 10th i had a scratch by a dog and and had another shot on 11th (Booster).

after this shot, i am having heavy headache, fever and bodyc ache wcihh i didnt experience with previous vaccinations.

Totally this is my 10th rabipur vaccine.

1. considering my history of vaccinations and current symptoms i have, can you please revisit your assessment and let me know if i am out of danger.

2. should i go for another vaccination without any fear or should i stop because i am having these side effects.

3. are these side effects are normal? do you see any point of worrying in my case?

Brief Answer:
Take next dose also.

Detailed Answer:
HI XXXXXXX

Side effects can happen with any vaccine but these are not severe . Just make sure that the expiry date of Vaccine is not near by ( that is not in few days )
Meningitis is one of the possible side effects of Rabipur vaccine which causes Fever , headache . But all fever headache are not due to Meningitis . Meningitis causes neck stiffness which has to be checked in you .
I advise you to consult your doctor and examine for Neck Stiffness . If it's ascertained that you have meningitis then you shall require other medications for that .
Even then taking next dose of Rabies Vaccine ( not Rabipur but some other brand ) is recommended because the prevention of Rabies holds of great importance when compared to the side effects .
I just want to put a note here that there is no cure for Rabies, if you get Rabies it's a death sentence .

Hi doctor, I don't mind getting one more vaccine here in this case.

1. If I see that dog is vaccinated. Can I stop taking next shot?
2. Considering my history, this my 11th vaccine. I really didn't have any adverse severe reactions to my previous 10,vaccines and what I had was a fever or headache for a day including tiredness. do you think this time also it won't cause any problem? as of now I don't have fever but have headache and tiredness.
3. Wanted to ask you a question. Can a rabies virus that is dogs saliva survive on surface for more than 5 min? That is like if I touch rabid dogs saliva and say after 5 minutes if I touch my eyes and mouth without washing, can that be still infectious and cause rabies or it dies immediately?
doctor

Brief Answer:
Side effects can happen anytime

Detailed Answer:
HI

I didn't understood what you mean by seeing that dog vaccinated. Either you get information whether that dog is vaccinated or not or just watch the dog ?

Anyhow if the dog is vaccinated then no need of vaccination or if you can watch the dog for 10 days and nothing happens then there is no need for any Vaccination .

Coming to your second question side effects to any medicines can happen any time, I had given an example of a person who is taking the same injection every month for 15 years developed a serious reaction to that same injection once. So side effects and adverse reactions can happen any time.

Coming to your last question Rabies virus will not survive more than 1-2 minutes outside animal or man, but again it depends on the temperature, humidity also. So the chance of infection in the scenario explained by you is extremely rare but still not impossible.

Hi doctor without taking chance I took rabipur today... This is my 2nd dose as I had my first one on 11th of sep... Thus completes the vaccination. As you aware al have also had two shots on august 25th and 28th. I hope that there will not be any adverse reactions and hopefully I will be fine. As ofnow I have headache and tiredness.
I also cjekced that dog and it seems fine.

1. Do you foresee any chances of over dosage adverse reaction as I have tolerated rabipur once.

2. If I were your patient, would you have recommended me to have two more dose though I had vaccinated 12 days back.

Please answer the above questions

Thanks
doctor
Answered by Dr. Divakara (1 hour later)
Brief Answer:
Probability of adverse reaction seems very less

Detailed Answer:
HI

Good that you got the Rabipur .

As per my opinion the chances of any major adverse side effects is very less , minor side effects like light fever, body ache , headache may happen but it will subside .

All the above answer given by me including this one is given with the intention to help you and thinking that you actually are my patient .

I wish you good health and stay away from dogs .

If you don't have any further queries kindly close the discussion and rate my answer,

Hi doctor.
Thanks
I have few more questions before closing this question.
Let's say I got an exposure today that is 36 hours after I received my second booster dose and 4th booster dose in a span of one month. In that case do I need to take rabipur again or my 2 instance of boosters that is 4 injections will suffice and provide the immunity against rabies infection.

Since my previous booster was on yesterday and previous to previous one was on 28th august, will my body have enough antibody to protect me at least from now on to a period of say at least one week?
doctor
Answered by Dr. Divakara (13 hours later)
Brief Answer:
Revaccinate

Detailed Answer:
HI

After completing the booster vaccination , any repeat exposure you should take Revaccination .

One more possibility is you can get tested for Anti-Rabies antibodies level in your blood and if it is in acceptable range then there is no need to revaccination .

But I wont test my patients for Anti-Rabies antibodies level. I just advise them to get re vaccinated again however early after last dose of vaccine they get bitten by dog .

Thanks doctor.
Do you know where is that particular test facility available in XXXXXXX so that I can get it verified.. I stay in a place where there are huge population of street dose is there. So I want to be sure...

Also more of a scientific question. If antibody titre is more than required and during that time if a person gets exposure, will antibodies neutralize the virus immediately or how it works?

Hi doctor, can the be answered quickly? Also I was browsing and came across this reply.

Could you please review this.

Brief Answer:
No data to support

Detailed Answer:
HI

Sorry for the delay .
I am not sure which centre in XXXXXXX does the rabies antibodies test as this test is very rarely advised.

You can go to CLUMAX DIAGNOSTICS and ask for Anti-Rabies Antibodies --IgG and IgM .

As per the link provided there is no data to support that . So it's up to you to believe .

Also I am trying to contact the Specialist person for Rabies disease itself , if in case of any change ( very unlikely ) I shall let you know .

Sure doctor. Please let me know once you receive any further updates from the specialist.

Doctor you don't believe I had another exposure on Sunday around 3pm when a dog scratched me to my finger. I think I have become affectionate to dogs. :)

Thus I asked a question. Today morning I received one dose that is at 11am. 40 hours after my 3rd exposure and will be receiving one more on Friday. Doctor also told me that I may not need it as I might have enough antibodies by now but he was also little doubtful. So to avoid any danger, I received it today.

Was I too late in receiving it?

Can I continue one more on Friday and finish this vaccination without having fear of any side effects?
doctor
Answered by Dr. Divakara (5 minutes later)
Brief Answer:
Go ahead and finish it .

Detailed Answer:
HI

Yes go ahead and finish it off .

Yes the dog scratching your finger is not actual exposure but again it's dangerous to take risk especially with Rabies .

Don't worry about side effects , nothing major will happen .

Hi doctor. You had a chance to talk to rabies specialist on my exposure and dosage schedule and also on the need of repeated vaccination. Please let me know if you have any information on that. It will ne helpful and really ease my fear. Thanks
doctor
Answered by Dr. Divakara (43 minutes later)
Brief Answer:
Only 1 booster dose

Detailed Answer:
HI

Yes I enquired with the specialist . He said if after booster dose any exposure happens ideally the victim has to get tested for protective antibody levels in blood . This is the same test which I had advised to get it for you .

If that protective antibody levels in blood is in normal range then no need for any further booster , but if the person cannot get tested then only one Booster Injection is enough and not 2 .

But he has not given any evidence for this information. He has said it is as per his trials and research .

So doctor I had completed my second booster on Saturday morning. Got exposed again on Sunday afternoon. Received the first booster on Tuesday morning that is 36,hours after the exposure.

Totally on august 25th to sept 16th I have received 5 doses. First set on Aug 25th and Aug 28th, second on sept 11th and 13th and third set of one injection on sept 16th..

Should I stop or should I go and get one more which marks the third cycle complete or stop vaccination..

Answered by Dr. Divakara (3 minutes later)
Brief Answer:
NO need for further vaccine

Detailed Answer:
HI

As of now no need for the second dose of vaccine .

In case of further bites you need to get IgG Anti-Rabies antibodies level test done and depending on the antibody level further vaccine is required or not will be decided.
