Question: Hypothetical Case 84 year old man, suffered recent stroke, does not drink or smoke, was active until stroke suffered last year. The patient suffered from heart blockages; hypertension and melanomas. Prescription drugs Cardizem (Antiangina agents) 180 mg/day once per day; Coloxyl (Laxatives) 120 mg/day once per day; Duride (Antiangina agents) once per day; Panamax (Simple analgesics and antipyretics) Three tablets per day; Pramin (Antiemetics, antinauseants) one tablet per day; Slow K potassium chloride (Oral and parenteral electrolytes) two tables per day. Query What would the potential drug interactions / contraindication for the drugs be / what would the medication treatment goals be?

Brief Answer: Pramin and Cardizem can be avoided Detailed Answer: Hi, Thanks for your query. Sorry for the delay in my response. Considering the history and the medication the patient is on I would postulate the below ideas. There are no interactions expected with the drugs being taken but together or alone some drugs can affect the patient considering the CVS (cerebrovascular accident) he had. Combination or both antianginic drugs may not be needed. If the condition permits taking off cardizem from the patient is a better option. The blood pressure lowering side effect increases when it is in combination with the Duride. Low blood pressure is not good for a post stroke brain. A blood pressure slightly more than normal levels is essential for a better perfusion through the brain matter for healing of the ischemic areas. Substituting Pramin with other prokinetic drug is good. Mataclopramide in Pramin is known to cause dyskinesia, extrapyramidal disorder, movement - uncontrolled or slow, nervous system disorder, headache. These symptoms can add up to the stoke defects. Also Potassium is known to reduce the BP and also reduces the platelet function. This could increase the risk of restroke. If the Potassium supplementation is necessary as to face the risk, continuing is permitted. Hope, I answered your query. Regards,
