Brief Answer:
Surgical treatment by orthopedic surgeon recommended.

Detailed Answer:

Hello,

Your situation is a bit difficult and aggravated since you have tried all nonsurgical and some surgical treatments.
I can recommend you to visit the orthopedic surgeon and he will treat surgically the cause of pain.
He can perform microdiscectomy.

In cases where the sciatica pain is due to a lumbar disc herniation, a microdiscectomy—a small open surgery using magnification—is the most common surgical approach. In this surgery, only the portion of the herniated disc that is pinching the nerve is removed—the rest of the disc is left intact.
A microdiscectomy is generally considered after four to six weeks if severe pain is not relieved by nonsurgical means. If the patient’s pain and disability are severe, surgery may be considered sooner than four to 6 weeks.

As a general rule, approximately 90% of patients will experience relief from their sciatica pain after this type of surgery.
According to the cause of it you may need a laminectomy.
In cases where sciatica pain is due to lumbar spinal stenosis, a lumbar laminectomy may be recommended. In this surgery, the small portion of the bone and/or disc material that is pinching the nerve root is removed.

Laminectomy surgery may be offered as an option if spinal stenosis causes the patient’s activity tolerance to fall to an unacceptable level.

Hope I have answered the question.

Thank you very much for your answer to my question about the sciatic nerve pain. I definitely will look into it. I have not consulted an orthopaedic surgeon. I have been dealing with this pain and have seen many doctors over the period of about 10 years. My question still remains; is there any medication that can temporarily relieve the pain when it is at its worst? The tramadol helped but made me extremely nauseous.

Brief Answer:
Wishing you will get well soon.

Detailed Answer:

Hello,

I find it very difficult to find relieve without surgery due to severity of case.
Firstly try not to do physical activities that trigger it and holding weights too.
You can use hydrocodone and acetaminophen since it is a narcotics medication that works by blocking nerve transmission.
Also, continue with steroid injections at spine.

Wishing you will get well soon.
Best regards.
