In clinical practice, no single lab test is used to diagnose insulin resistance syndrome. Diagnosis is based on history and exam and corroborated with laboratory tests.
Usually we start with
- blood glucose level which may be fasting, random, or a glucose tolerance test
typically a fasting glucose level is a good place to start
- AIC (glycohemaglobin level). This measures the average of blood sugar over a 2 month period.
- Fasting Lipid Panel (total cholesterol, LDL, HDL, Triglycerides)
Insulin resistance syndrome typically has elevated LDL-B levels , high triglyceride levels, and low HDL levels.
If these tests are abnormal, then a fasting insulin level might be done, which gives a measure of the degree of insulin resistance.
If a person is overweight and inactive, no further testing is typically needed to look for the cause of the insulin resistance. However, if that is not the case, then other tests may be considered.
