Is There Any Relation Between Spastic Paraparesis And Myelopathy?

Question: I am doing a f/u of a question I asked about spastic paraparesus and myelopathy - what is their relationship? My son is being treated for these 2 and has been slowly losing his ability to walk smoothly in the last 5 years. He is treated at the Montreal Neurological Institute and takes baclofen plus physio exercises. Of course, we are wondering if there is any chance of healing that we do not know about already?

I have read your query and understand your concern.

Myelopathy literally means pathology/disease of spinal cord.

Based on the segment affected,it can give rise to symptoms of spastic paralysis and hyper exaggerated reflexes.

This can over a period of time lead to difficulty in walking.

Myelopathy has numerous causes.

It would have been better if you would have enumerated the cause of the myelopathy by attaching/uploading the scan or diagnostic reports of your son.

Central conduction time-CCT,which measures the time required for the nerve impulse originated in the brain to reach the spine.

This time is measured by Transcranial magnetic stimulation-TMS.
This test can accurately define the severity and scope of recovery of myelopathy accurately over a period of time.

Chances of recovery or healing highly depends on the cause of the myelopathy.

Progressive weakness with increased difficulty in walking over the last 5 years is not good sign of recovery in your case.

In general compressive myelopathies have better outcome than other types.

So in order to elaborate the chances of healing in your son's case with certainity,i request you to upload few diagnostic reports of his in your follow up query if possible.

I would have to obtain copies of the tests you are speaking of through the neurologist at the Montreal Neurological Institute and that may take a while. We are unaware of any "cause" and my son tells me that the doctor has suggested that the origin of the problem may have been from many years ago (congenital?) and that it has just shown up now. There has also been some evidence that this problem is related to intellectual slowness. My son has special needs although he did graduate from high school, drives a car and plays chess.
I have requested to have a meeting with my husband and the neurologist and should hear when that could be in the next week.

That seems a good option. Please have a follow up, acquire scan reports for future purpose at the hospital.

Congenital lesions of spinal cord tend to have poor outcome overall.

The cognitive and intellectual deteoration in your son's case has not been of any severe degree since he finished his graduation and quite capable of playing mentally skilled games like chess.

Congenital myelopathy seems to be less likely possibility in your case.
It should most likely to be a developmental cord disorder in my opinion.

Any how specific comments can be made based on the test reports.

I will give some news after I speak with the neurologist here. Surprisingly. I have also had a reply from a Dr. XXXXXXX XXXXXXX neurologist from your company. This is the first time I have used this website and did not realize how to work it. Hopefully you have access to his responses also.
