Is There Any Serious Issue Having Chest Pain With Diabetes ?

Question: My age is 35, from last three years I am diabetic, my diabetes is under control(hba1c 6%). Now from last 10 days I am feeling chest pain, I was feeling like acidity and my stomach was little bit disturb also. I have upper chest and back pain also. And i was feeling throat tightness. I visit the doctor and after check up she give me Acidity medicine.
After 3 days in the morning time I was sitting in lounge and suddenly I observed my heartbeat is going too fast and it was shocking for me and i was feeling lightheaded. This continued for near about 1 minute and after 10 minutes i was normal but this happened three times.
That time I don't have significant chest pain or may be no pain.
After one hour I went to hospital emergency. They have done blood pressure heart rate monitor and ECG. As per doctor all of these were normal but they told me if you have family history of heart disease then you have to go for cardiologist. My father was a heart patient and discovered in his late 40s. After visiting cardiologist at night doctor recommend me some blood test. which are CKMB and troponin . and my results were
CK-MB 0.58 ng/mL
troponin 0.012 ng/mL
Results were normal as per doctor, but due to my family history and my diabetes (i think)he gave me another test. which was eco test for heart. and that was also perfect as per doctor. He told me there could be chance of acidity or anything related other then heart. i came back and now after 2 week again i have stomach disturbed and chest pain in left side. which i feel more today morning. i have no idea where i should go. I have stomach gass issue but i dont have such kind of pain before. pain some time i have when i move in a specific direction and sometimes i have without much movement. and it goes little bit of my upper back. i still have the fear of heart disease. or any other serious issue kindly guide me.

Hello,

Although you have diabetes and positive family history of heart disease at an early age, the negative troponin tests reassured the doctor that there was no acute coronary syndrome. The negative echo is very much against heart disease as well. What concerns me a little bit is that you've had symptoms resembling arrhythmia. Having arrhythmia with a structurally normal heart is not very common. Despite that checking with a 24-hour Holter for ECG is recommended. If an arrhythmia is detected during this time then we will know. If during this time you have symptoms but no sign of heart disorder then the heart disease can be safely excluded.

Your symptoms indeed seem like an acidity problem. I would also suggest an upper abdominal ultrasound scan because sometimes the gallbladder may cause symptoms resembling stomach disorders. If the symptoms persist then an upper gastrointestinal endoscopy may provide more clues.

Such symptoms usually resolve with medications against acidity like the proton pump inhibitors.

Thanks for the detailed reply.. After cardiologist checkup. What i understand that heart palpitations might be due to low blood sugar... But chest pain was not fully recovered and yesterday i visit the doctor again. That was internal medicine doctor. He checked all of my history and told me that you have musle problem is between ribs. And he gave me one gel and one medicine for pain i think.. And said dont worry.
But sir my question is if this pain is muscle pain then why it comes and goes automatically. But doctor told ne when u turn to a specific angle then only it will come. Some how he is right. But everytime this pain is not comming by movement.

Musculoskeletal chest pain typically worsens with movements. Sometimes you may find a tender point on your chest. If the heart, lungs and vasculature disorders have been excluded as pain causes then it's usually nothing serious.
