Question: Is there anything new for nonallergic rhinitis and a nose that runs clear fluid constantly? My allergist said it may have an allergic component also. I have been taking Allegra (helps a little bit I think), and using ipatropium bromide nasal spray to stop the running (for an hour or so). My allergy testing showed that I was not allergic to very many things.. just the usual dust mites, dogs (we don't have one), molds and a few other things. I have tried many different treatments my allergist recommended such as nasal rinses (large volume lavages 4 times a day) , steroid nasal sprays, and mattress and pillow covers. None of this did any good and my nose still runs like a faucet! I read about spinal fluid leaks and it scared me! (But I have no other symptoms such as headache). If I ever forget my hankie and kleenex, I have to go back home and get them. Sometimes I have even dripped on people or the floor--very embarrassing. I am 75 years old and in very good health ( have well controlled hypertension). My nose has been running since I was around 50. Any suggestions? (meds: lisinopril, metoprolol (very low dose), Allegra). Your input would be vastly appreciated!

CSF rhinorrhea is associated head injuries and presence of intracranial space occupying lesions like tumors. The Specific test to differ between the two is the presence of Beta 2 transferrin. Beta 2 transferrin is absent in Allergic rhinitis. You may ask your doctor to provide you with this investigation to rule out the above possibilities.

The best combination that works for my patients with allergic rhinitis is
Tab pseudoephedrine + cetirizine twice a day
Tab montelukast 10 mg once at night
Nasal sprays containing olaptadine and cromolyn
1 spoon of turmeric in hot milk at night
Tab serratiopeptidase 10 mg twice a day for a week

You may discuss these options with your doctor, who can then prescribe them accordingly to see if it works for you.
