Question : can I take two 25 mg of promethzine after taking 1 and half tramadol 5 hours ago have terrible migrane with nausea.

If the Tramadol you have is immediate release (not extended release) and if it is not a repeat dose (i.e. you only took 1 dose today), then after 6 hours, it would be safe to take the promethazine. The combination of the two has the potential to increase the risk for seizures.
