Question : I am evaluating the use of 5-htp with Tramadol....my MD put me on Effexor 37.5 mg to treat SE of exemestane (joint and muscle pain, night sweats) ...I took it for a little over a month but had SE of constant headache, nausea, and horrible dreams, so weaned myself off, which took several weeks (even at this low dose) during which I was miserable. I am taking Tramadol 50 mg twice a day for hip, back and hand pain, caused, I believe, by exemestane. I had hoped the 5-HTP might help with the pain and associated mood issues, but see that it may be contraindicated with Tramadol because of effect on serotonin...MD did not think effexor and tramadol together would be a problem because of small dosages of both...but I could not tolerate effexor. I imagine you will tell me to discuss further with my MD, but this box popped up so I thought I'd give it a try:) HX - I am 64 yo, retired RN....had stage 1, grade 3 breast cancer, er+ pr+, lumpectomy in 2/2013, clean margins and sentinel node biopsy clear, 37 radiation treatments.....all follow-up has been ok...some osteopenia that MD says may be age related...cancer markers have been okay.....but I feel like crap with fatigue and especially joint and muscle pain. I walk, attend exercise class and do gentle yoga twice a week. I'd like to be able to do the full 5 years of exemestane, but feel I am trading off on quality of life. Would appreciate any comment on use of Tramodol 50 mg Bid with 5-HTP 100 mg daily......and any other words of wisdom. Thank you for reading this.

In addition to my original question, I am particularly interested in use of supplements, dietary changes, etc. which might positively impact the general feeling of crappiness I have......using, and getting off of, Effexor was a horrible experience. I don't like taking Tramadol, but it does help, so I do. I tried, and could not take Vicodin and Tylenol #3...I felt doped and sick on both. So far the Tramadol has been a life saver, but I don't want to take it forever (maybe just until I finish the exemestane in 3/2018?)....but I understand people often have difficulty getting off of this med, too. ....I do the right dietary stuff (mostly plant based, Mediterranean style diet) In addition to meds mentioned, I take multi vit, calcium 1200 mg, vit D 5000 mg, turmeric capsule, krill oil capsule...all daily.

I can understand your concern. Your main issue is any risk of drug interactions between Tramadol and 5-HTP. Tramadol is a good analgesic drug and is known to increase serotonergic neurotransmission in brain. 5-HTP or hydroxytryptophan is a precursor of serotonin and is used in treatment of mood disorders especially depression. Theoretically these drugs if used together can cause or rather I'd say increase the risk of serotonin syndrome. But considering your doses 100 mg (50 bid) Tramadol and 100 mg 5-HTP the risk is very low and is almost negligible. So I don't think there is any serious risk to side effects if these drugs are used together. I have used these drugs to few patients of mine and none reported any significant side effects.

Coming to your second query. Yes you can go for dietary change to help in improvement of these symptoms.

- Take high protein and complex carbs and low fat diet. This will help in increased serotonin in brain and thus will improve your mood.

- Start taking Vitamin B12 supplements along with your other drugs. Methylcobalamin should work better. Discuss with your doctor for this drug also.

- Take diets rich in OMEGA-3-FATTY acids. Salmon, Sardine fish, walnuts, XXXXXXX nuts etc are good source of these fatty acids. This will help to reduce the fatigue as well as will improve your mood.

- I'd also recommend you to take at least a cup of green tea daily. This will also help in improving your activity levels.
