Question: I have unusual weakness in my legs, to the point where I often need help rising from a chair. Increasing severity over the past couple months. I have a lot of fatigue, very tired after exercise.
Normally I am very fit and exercise moderately several times a week.
I have chronic imbalance from a neck injury which has increased, and an irregular gait at times.

I can understand your concern. As per your history I think it may be osteoarthritis. Other causes which can cause such symptoms are injury or blunt trauma, rheumatoid arthritis, sprain or strain, overuse, lifting heavy weight, peripheral neuropathy or rarely motor neuron disease. Let me know please whether do you have history of injury or backache, difficulty in movement only in the morning? Kindly follow up with detailed history.

I will suggest to do x ray knee joints and MRI back including hip joints. Along with that you can do RA factor, ESR, CRP, Anti CCP, serum calcium, B 12 level, EMG-NCV to rule out other possibilities. Treatment mainly depends upon the underlying condition. You can take tab. metaxalone 400 mg plus diclofenac 50 mg. As these are prescription medications so consult with your doctor. You can use knee cap and lumbar belt. Use support sticks to stand and walk. Don't give much pressure. Do regular physiotherapy after consultation with physiotherapist.

Chronic imbalance and gait imbalance may be due to BPPV as per your history. You can use tab. vertin 16 mg during acute episodes. I think this will clarify your concern.
