Question : I’ve been experiencing knee pain for the past few days. After consulting with an orthopedic doctor here, my CT scan report indicates that there is bone growth under the knee. The doctor has advised either surgery or an injection for temporary relief.

As I’m planning to travel to XXXXXXX in July, I intend to consult with my family orthopedic specialist there for a second opinion and possible treatment. Based on this, I am planning to apply for six weeks of medical leave.

I need to submit the FMLA application form for taking 6 weeks leave. i can provide my medical summary report for reference.

can you please help me on getting the form signed?
