Question: Last evening I felt some discomfort in my scrotum (right testicle) and overnight the area around or the right testicle itself became swollen (twice the size of the left testicle area) and caused pain when touched. This continues this morning. The pain is not any greater, but does persist, and the swelling seems to have found its maximum for the time being. I do have congestive heart failure (wear a pacemaker/ICD - EP issues - since 2011). I also have evidence of an abdominal hernia (naval area is extended). What could be the cause of this testicular discomfort and how may I treat it?

Hi.
Thanks for your query.
Noted the history well and understood the concerns.
The history is suggestive of one of the following:
Right side Epididymo-orchitis: infection of the testicle and epididymis.
Torsion of right testis : usually very painful.

This can be diagnosed by the following:
Urgent Ultrasonography and color doppler of the scrotum.
Relevant tests of urine and blood.

Hence I would advise you to consult your Doctor or visit ER immediately considering the cardiac problems that you have.
You may please give scrotal support.
Take an anti-inflammatory that suits you or have been advised by your Doctors recently.
Further management will be as per the cause found.
