Question: From MRI report "tear of annulus fibrosis left paracentral region l4-l5." Injury has existed for over two years with the most recent MRI report stating "tear ..." What are treatment options for this condition? Patient has had shots. Rest. Physical therapy. However, pain persists.

From the description of the MRI findings - it seems that you have a peripheral tear (Rim lesion) of the annular disc.

An annular tear, also called a herniated disc or ruptured disc, occurs when the outer wall of an intervertebral disc becomes so weak, thin, and dry that it develops a tear.

The inner disc fluid, called nucleus pulposus, can leak into the spinal canal and press on nearby spinal nerves or the spinal cord causing the pain and discomfort.

Your body recognizes that the extrusion of this nuclear fluid is abnormal and it makes attempts to reabsorb the fluid and allowing the disc repair spontaneously. In such cases surgery will not be necessary.

In case, the above measures including physiotherapy and pain shots are not helping, and the severity of the tear is very bad, then there is a option of surgical repair of the torn disc ( Annular disc repair ) done by minimal access spine surgery.

You might consult a spine doctor specialized in endoscopic repair, for proper treatment and advice.
