Question : My 73 yr old wife was prescribed PREMARIN VAG 0.625MGCRE WYE dosage 0.5 Gram intravaginally daily. Within an hour of applying the cream she experienced a severe burning sensation and also lower abdomin - two places on either side of her abdomin (I'm guessing - ovaries) also had this burning sensation. Is this a known reaction? I advised her to quit using it...but thought it might be a coincedent. Should she continue using it?

First of all do not panic.

In my opinion she was prescribed all the medicines for itching and or discharge from vagina.
Please confirm.

At her age, this can happen due to senile vaginitis.
Medical treatment suggested to her is right.
But burning sensation can occur after application of estrogen ointment if there is infection and cracked vaginal skin.

1. Has she done gynec check up?
2.Has her doctor suggested to get USG done?

I will suggest to get her gynec check up done.
Avoid premarin ointment for a week.
Please seek prescription support from your treating doctor for phytoestrogens tablets.

For lower abdomen pain, I will suggest to get antispasmodic tablet like Hyoscine as needed.

Thank you Dr. Neurgaonkar. My wife has had gynec check up and has also had a USG, as well as a urinalysis and all appeared normal. Her Dr. said it was common for thining of the vaginal walls at her age. (or something to that effect.) We will talk with her Dr - or Nurse Monday about your suggestion for Phytoestrogens tablets and Hyoscine for her lower abdomen pain.
Hopefully I can get back with you after speaking with her Dr. or Nurse.
Again, thank you for your suggestions and hopefully the medications you suggested will be approved.

Your treating doctor has rightly said that vaginal skin becomes thin due to age. This is called as Atrophic senile vaginitis.

Use of lactic acid containing hygiene wash during bath time and keeping private parts clean and dry is advisable.

Phytoestrogens are plant based compounds that have positive effects of estrogen hormone on vaginal and urinary tract skin, bones and lipid profile.
At the same time there are no harmful effects on uterus, endometrium and breast tissues.
