Question : My 8 months + daughter is having Rubbing/Grating sound in her knees and we can feel lower bone movement in knee moving sideways since past 3-4 weeks ( we first observed this when she had viral Fever 3-4 week back other there was no such sound earlier).in Normal course she is playing moving legs , started crawling, started Started to standup by her own by taking support .
We visited a Pediatrician Ortho and he first suspected that it's patella of knee that could cause however later when he observed the knee ( we irritated our daughter) he updated that this is not related to patella and could be some ligament( he requested X-rays )
He also said baby has dimples on both knee and patella seems to be small.
He ordered X-ray of Pelvis and knee.
On seeing X-rays he told that baby has a mild hip dsyplacia on the left side however he said nothing is required for the moment and we will followup with after 2 months and lets see the baby development. On Manual Examination earlier he cleared that there was no problem in Hips.
He told even if it remains then the kid needs to be sit in a harness to correct it and there can be no intervention till 3-4 years.

1) Can you please confirm that the dimples in the knee are normal for this age.
2) What could be possible cause of this grating/rubbing sound in both knees and movement of bones sideways.
3) Could this grating/rubbing could cause pain because out of 10 instances for example we see in 1-2 instances she is already irritated and shout and then when we take her in our lap or give her something to play she seems to be having no issue.

                                    Brief Answer:
Please upload an image as well as the reports
Detailed Answer:
Thanks for using the Ask a Doctor service.
I have gone carefully through the provided details. I would request you to upload an image of the knees (including the dimples) as well as the report of the X Ray (if possible the image of the plates after placing against a bright background, could be after opening paint on a computer). Regarding your queries:
1. I would need to look at them to be able to comment.
2. Possible. Possibly not a cause for concern. Can comment after looking at the report.
3. Do not worry about it. Will comment after checking the images.
I will await your inputs.
Regards

No need to be apprehensive
Detailed Answer:
Thanks for the images (both X Rays and photos)
1. I would consider the dimples normal and not a cause for concerns.
2 and 3. Possible. An orthopedician can examine the child and comment whether the joint cartilage seems fine. If they feel fine, there is no cause to worry.
Regards

                                    Brief Answer:
Explanation provided
Detailed Answer:
It is possible that the grating sound could be due to damage to knee cartilage and in that case, it could be painful. An orthopedician could examine and comment.

Thanks we have already shown to P.ortho and he has examined her 1 month back and however he told it could be lose ligaments.
how can i have confirm the diagnosis because this sound even though come several times in day when she is in our lap or when we have picked her up , however we are not able to figure out whether she is in pain or not because we observe that when she is irritated(like for sleep or breast milk) she make this sound + shouting.
she crawls , stand on her feet for 15-20 mins or more with herself holding some support.she move here legs fastly in bicycle position while playing.
So all these are confusing me.

                                    Brief Answer:
No need to worry
Detailed Answer:
As of now, do not get concerned. Review with the orthopedician subsequently and see what he feels. Since he feels that it could be due to loose ligaments, essentially it conveys that he did not feel a cartilaginous problem to be in place (if he had found it, he would have expressed it). In such a case, stop worrying, wait and watch.
No active intervention or investigation needed at present. The motor milestones are proper and do not seem to be in jeopardy. So, you need not worry.
Regards
