Question : My chest has been hurting last few days. I went to ER Monday In fear of a heart attack, I was not having one. But my chest continues to hurt and increase when I lie down on my side while trying to sleep. My pulse is normal and blood pressure was 130. I aam male 42 with no history of heart issues or family. I don’t smoke and rarely drink. But the pain is legit and scary in my chest. I’m trying to get in with a cardiologist but that’s not till who knows when.

Considering the above pain characteristics, your symptoms are likely to be of an extra-cardiac origin. Coming to this point, as the preliminary ER tests haven't been able to detect any acute cardiac issues, it is necessary to further investigate in alternative directions.

A muscular-skeletal disorder (such as costochondritis) should be sought in case suggesting symptoms and signs are present (local edema, tenderness on palpation) is present.

Additional medical test are advisable:

- complete blood count with leukocytes differential, ESR, PCR and chest X ray study to rule out any underlying pulmonary inflammation/infection;
- backbone X ray study to investigate any possible degenerative disorder
- a chest CT scan could provide additional medical information.
