Brief Answer:
Require further evaluation. Follow up with reports and few more details.

Detailed Answer:
Hello,

I have gone through your query and here is my advice.

As per your history, as he had undergone many investigations. Kindly upload report images, so I will get better idea. Still kindly follow up with a bit more information.

Please let me know does he have cough, weight loss, loose motion, blood in stool, abdominal pain, decreased appetite, headache, leg pain, joint stiffness mainly in the morning?

For further assessment, as per the history, you may require positron emission tomography (PET) scan, HLA B27, RA factor, thyroid profile, venous Doppler study, lumbar puncture, repeat blood and urine culture sensitivity, peripheral smear (sample to be collected at time of high grade fever) after detailed medical examination and physician's opinion. Before that kindly follow up with detailed reports.

Treatment mainly depends upon the underlying condition. Till then for fever, you can take tablet acetaminophen 500 mg every 6 hourly. Take balanced diet and keep him well hydrated. Do regular blood pressure monitoring.

Hope I have answered your query and let me know if I can assist you further... Thank you.

Regards,
Dr Shyam Kale
