Dear doctor,
In my last question I had mentioned abt,my mother,
1.My mother,72,nondiabatic,suffered from unstable angina,in feb.
2.I will upload the reports.Angiography shows
blockages,
3.2d echo done 3 months ago showed 60% pumping rate .
4.My mother was undergoing ayurvedic treatment for 3 months.No oil,no rice,no junk food at all.mellate,sprouts ,dal included in food,
20-25 min walk,
5.Pumping rate today is 68%.report will b uploaded lster.
6.Should ee go for engioplasty or CABG hurriedly ??is it really necessary for such hogh pumping rate?I m cpnfused whether to continue ayurvedic treatment or to go for engioplasty or CABG?

I have seen the attached Echo report, and the left ventricular ejection fraction is quite normal indicating that the heart is able to work well despite the increase in the size due to high blood pressure for many years.
I will answer your questions in reverse order.
5. There is no urgent need to do CABG or Angioplasty, to do these, we must first determine the degree of blockage and the site of blockage in the arteries that supply blood to the heart.
Relax, but please do get an angiography done. Angiography will show us clearly where the block is in the blood vessels and it will also show how much it is blocked.
The treatment will depend on the degree of block and the location of block.
If your mother is put on blood thinners like warfarin or low molecular weight heparin along with cholesterol reducing medicines, the block might have cleared off.
At this point in time, my suggestion is to go for coronary angiography, and decide the future course of action based on that. There is no need to hurry.
4. Ayurvedic treatments do not help much, yes, reduction of oil and junk foods will reduce cholesterol to some extent, but if the cholesterol level was more than 350 then a statin like atorvastatin should be started to reduce the cholesterol so that arteries do not get blocked further.

I have uploaded engiography report right now .This engiography was done along with 2d echo on 14th feb,
2.It is clear that 3 blocks ,99%,70%and 50%.99% in right
artery.
3.We consulted cardiologist in XXXXXXX He asked to go for engioplasty of just one block in right artery ie 99%.
He said though not emergency ; that block may create a problem in future,He said CABG not require as other 2 blocks are 70%and 50%.( in graph it is seen 80% but the doctor saw cd and told us).They are in left ventricle,he said to do plasty is bit difficult that side.but 99% block can be treated for engioplasty,
4.She is ready for plasty,but I am extremely tensed as the block intensity is 99%.I know that the cardiologist is a skiiled one,but afraid .because right now she is alright
and following changed lifestyle sincerely.
5.We are planning to go for plasty in next week.will it be risky??pl answer asap.want to take a ,decision

Angioplasty is highly recommended, it is much safer than CABG.

Thank you for uploading the full report, I have seen the attached angiography report.
Your mother has blockage in more than one site and since some blocks are more than 90%, she will need angioplasty.
CABG may not that useful because the blocks extend quite far down the arteries.
Angioplasty is best suited in your mother's case.
My advice is, please go ahead with the angioplasty in week.

1.Cardiologist suggested 2d echo before plasty.
yesterday we did it and found that the heart pumping rate increased to 68%.She is taking blood thinning and cholesterol reducing tablets.Is this incresed heart rate attributed to that medication??
2.Is the engioplasty of 99% block in right artery is safe??
3.She is not having any pain right now.Is there any relation between the incresed heart pumping rate and blockages in her specific case?
4.I heard that ayurvedic medicines develop the colaterals which increse the pumping of heart,Is this true?
68% Ejection fraction is good, but angioplasty is still needed.

Ejection fraction only tells us that the heart is pumping well, that does not show if the block has reduced.
Cholesterol reducing tablets and blood thinners would make the plaque size less but the plaque will remain and can get blocked in the future that is why angioplasty is recommended.
Yes, angioplasty is safe even if the blockage is 99%.
Not having any pain is a good sign and it indicates that the block has stabilised so it is safe to do angioplasty now.
No, ayurvedic medicines do not develop any collaterals, there is no evidence to conclude that from any ayurvedic treatment.
Please go ahead with angioplasty as advised by the cardiologist.
