Hello,
You seem to have eczematous rash on scrotum. I suggest you to use a moderately potent topical steroid e.g either fluticasone propionate 0.05% cream or triamcinolone acetonide 0.1% cream, twice daily. These are prescription topical steroids. In addition to that I suggest you to take an oral antihistamine e.g Cetrizine 10mg tablet once a day for symptomatic relief from itching.
