If the swelling/bulge comes on coughing and is reducible by pushing in or on lying down then this is most probably a hernia of the abdominal wall.
The pain is usually indicative of obstruction in the hernial sac which may be fat alone or with intestines.
In such a case it is better to take her to ER (Emergency Room), clinical evaluation, examination, and investigations like X-ray of the abdomen in standing position, Ultrasonography, and tests of blood.
This will give an appropriate diagnosis and gives a plan for management.
