Question : My father who is 86 yrs old has been found very high SGPT andSGOT level. He is a high BP patient and on regular medicine to keep it under control. He follows a strict diet of no oil food. Then why he is having the levels so high.

High SGOT and SGPT levels suggest gross impairment of liver function. This can be due either causes in the liver or due circulatory causes.
Liver causes are
1. Inflammation as seen in hepatitis which is commonly viral in origin.
2. Cirrhotic changes which is a fall out of infection of toxic injury.
Investigations for this are
1. Viral markers
2. USG of the upper abdomen
3. Coagulation profile including PT/INR
4. Upper GI endoscopy.
Cardiac causes
1. Right sided heart failure due to long standing BP can also lead to cirrhosis of liver thereby altering liver function.
Investigations for this are
1. 2D echocardiography
2. ECG
Depending upon the cause ,appropriate treatment needs to be commenced.
