There may be multiple differential diagnosis of your complaints of cramps,twitching,tremors, weakness and parasthesia.
If I receive such patient in my hospital,I immediately procure a plain CT Head(NCCT) to rule out a stroke.Stroke may present as a facial deviation,slurred speech or arm weakness.
Let me know if you have any risk factor like heavy smoking,alcoholism,hypertension,diabetes,family history,obesity or sedentary life?
Signs of UMN lesion include weakness, hyperreflexia and increased tone.
Signs of LMN lesions include weakness, atrophy,hyporeflexia or absent reflexes.
Once UMN lesion has been ruled out by a brain scan,I would go for a MRI of cervial and thoracic spine(Plain and contrast s.o.s)
It will rule out prolapsed inter vertebral disc(PIVD) and compression radiculopathy.
If any Intervertebral disc is partially or completely prolapsed,the disc bulge obliterates the foramina through which the spinal nerves comes out.
This would cause pinching of nerves and sensory and motor deficits thereby.
This can be substantiated by Nerve conduction study and electrophysiological studies.
You also need to test for serum calcium, serum vitamin D3, serum electrolytes, blood sugar fasting and PP(to rule out diabetic radiculopathy),VDRL, HIV, PTH (Parathyroid),serum TSH, X-ray cervical spine (to rule out cervical rib),serum vitamin B12 levels and CPK enzyme levels to rule out the causes which have a similar symptomatology.
