Please Advise Me The Desirable Levels Of BP For Diabetics Of My Age 79

Dear Doctor,
I am 79 year old and vegetarian.I have hypertension (mild) since 1980.At present I am taking Betacard 50 mg from May 2017 and Amtas 2,5 mg from Feb 2021 after breakfast .I am also diabetic since 2012 .My Hb A1 c is 6 and at times 6.5 or 7.Fasting is around 126 and pp is around 160 or 170..I am taking glycomet sr 500 mg twice a day.
I am herein forwarding the bp readings taken fora week on omron digital meter at home.I found when ever i take Amtas 2.5 mg in the evening also , the readings are normal and when i discontiued iit, again they are going up.I also found that when ==ever i take Amtas in the eveining , the next day my diastolic is going down to 70 or 72 and i physically feel that my legs are light. So to avoid this i am not taking the amtas.Ofcourse my doc did not ask me to take amtas in the evening.Just to reduce the bp i am at times taking and then discontinuing it.
Kindly advise whether my bp readings are normal .Is it necessary to take amtas 2.5 mg in the evening also.if so how to prevent diastolic getting low the next day? Or do I have to change the medicine ?
Please advise me the desirable levels of BP for diabetics of my age, since i am not getting correct info rgarding this..i am waiting for your valuable reply

I have read your query and seen the reports.
Dr XXXXXXX is currently unavailable to address your query, therefore I have been assigned this query.

In elderly, blood pressure levels are usually on higher side and a level of less than 140/90 is generally acceptable. In presence of diabetes, a level of less than 140/80 should be the target. Accordingly you keep a regular watch of your blood pressure and whenever it goes above 145/95, Amtas can be taken twice a day under supervision for sometime.

Amtas is a safe medicine and does not cause any major side effect on long term use. Minor side effects include swollen ankles, constipation, flushing of face. It usually doesnot cause hypotension (blood pressure less than 90/60). It is also a suitable antihypertensive in diabetes as it doesnot increase blood sugar levels. So you can continue it. Nothing currently needed to increase diastolic blood pressure or to prevent lightness of legs due to Amtas. The lightness of legs could be due to its vasodilatory effect on blood vessels due to which it treats hypertension.

Just to add, Betacard (atenolol) can cause higher blood sugar levels, so check with your physician if it is a right medicine for you.
Low salt diet can be taken to reduce the blood pressure. Regular exercise also helps.

Dear Doctor,
I thank you for your prompt reply.I have one question. I am taking Amtas 2.5 mg morning after breakfast and after dinner also. Is it ok or do i have to take it around 7 pm in the evening. Kindly advise.

Thanks for your followup.

Amtas has a duration of action longer than 24 hrs.
Evening dose can be taken on most days a week. You can take it early in evening, 5 pm as BP is raised at 5 pm measurement. Sleep/rest seems to lower your blood pressure. So evening control of blood pressure might be needed in your case.
