Hello!
Welcome to Ask a Doctor service!
I passed carefully through your question and would explain that your symptoms seem to be related to panic attacks.
Anyway, it is important performing a cardiac ultrasound in order to examine your heart function and structure.
The fact that these episodes are not related to physical activity, is very reassuring that nothing serious is going on.
Nevertheless, I would recommend putting a Loop recorder, which is a device, similar to the Holter, but it can help monitor your heart rate snd register those episodes for a prolonged period of time ( months) and help explain better what is exactly happening during those episodes.
A cervical spine X ray followed by a cervical spine MRI would help investigate for possible degeneration of the vertebral column and may be explain the neck cracks.
It is also important checking your thyroid hormone levels for possible thyroid gland dysfunction and blood electrolytes for possible imbalances.
I remain at your disposal for any further questions whenever you need!
I would be glad to review your performed test reports for a second professional opinion!
Wishing good health,
Dr.Ilir Sharka, Cardiologist

My opinion as follows:
Detailed Answer:
Hello again!
Thank you for the additional information!
As you are wearing a Holter and you felt something those ectopic heart beats, I am sure that they will get registered by the machine and the doctor will be able to interpret it, knowing the time that they happened.
This will help clarify if it is benign arrhythmia or something more serious.
I would be glad to review your Holter report, whenever you will be able to upload it for a second opinion.
From the other hand, the fact that they occur only during sleep, could be related to acid reflux or just a normal reaction during REM phase of sleep (which is also called paradoxical sleep, the time during sleep when the heart starts to race, although the person is in deep sleep).
I recommend trying an antiacid before sleep and trying to regulate your sleeping hygiene.
Hope to have been helpful!
Wishing all the best,
Dr. Iliri
