Question: I am 34 yrs old 72''weigh about 220.

Today I am writing because about 7 months ago i ended up going to a brothel. I was on vacation with some friends. I messed around that night.... a few weeks later I started itching real bad all over. I dont know if it was guilt that I had cheated on my significant other or what it was but I was afraid to have caught something although I used protection. I went and got a full work up and all was in the clear. The itch continued and only got worst at night. I went to the doctors more than a few times. They drew blood, checked me for HIV 3 times within 3 months and all came back good. Then they did a last CBC and it came back that my monocytes and eosophils where slightly elevated. I think about this situation all the time and think that I might still have something. I also went to a dermatologist because i believe that the itch is due to scabies. He has seen me about three times and tells me that I have atopic dermatitis. Between my doctor and GP they have treated me for scabies 3 times and I still continue to have the severe itch. Since the itch started it only stopped once and has been ongoing. It affects my head, face, shoulders, under arms, chest, nipples, arms, hands, between fingers, groin area, thighs, knees, legs, feet, soles of feet, between myn toes, wrist, and ankles. I am losing it. just need some advice on what this can be. Can it be anxiety and stress causing all of this itching. I also have a slight rash from time to time and flaky dry skin. can anxiety and stress elevate monocytes and eosophils. im also a smoker so can this be a form of cancer like hodkins lymphoma, etc. I am a wreck as you can see any advice in regards to all of this would be great.

I am also afraid that this could be due to an XXXXXXX disease but my doctor and dermatologist think im going crazy. i got my liver checked out and all came back good.

Thanks in advance for your timely response.

Hello and thank you for the query.

First of all let me reassure you that I do not think you have any worrisome XXXXXXX disease or sexually transmitted illness going on. I have to say with confidence that stress is a very potent stimulator of inflammation. By inflammation I mean that a stress response can cause a multitude of symptoms including generalized itching, sweating, weight loss among others.

You have obviously had this on your mind and it is causing you a great deal of stress. It is not surprising to see that you have experienced the itching as I am convinced this is a stress response. You have seen your doctor and a dermatologist who both have reassured you that you are healthy and there is no evidence of a disease process at play.

I feel this is anxiety and stress related. What is important for you is to treat the stress response and I recommend the following:
1) Exercise at least 30 minutes every day including good aerobic exercise - this is a fantastic way to release stress and I know it will work for you.
2) Maintain good fluid and food dietary intake, eat healthy with a well-balanced diet.
3) Meditate, try yoga or other relaxation techniques to help maintain an inner peace.

Try not to continue to think about things for now. Work on making tomorrow and the coming days better. You are healthy and I know you will continue to be a healthy and vibrant person in the future.

I thank you again for the query and hope my response was helpful and informative. If you have additional concerns I am available to address them.

Sincerely,

Dr. Galamaga

What about the elevated monocytes and eosophils? Can that be related to the itching or an XXXXXXX issue?

And can stress really cause all of hese issues. So if i relax more and think less of it the problems should subside?

Hello again,

The elevated monocytes and eosinophils are not really specific for an infection.

Often at times we see that in response to a mild environmental allergy.

Please do not worry about that at all. BUT it could be reflective of the itching you have going on. I think maybe some benadryl two to three times daily would be advisable for you to see if this calms the itching down.

I really am convinced that stress is playing a key role here as well. Focus on relaxation and I am confident your symptoms should subside.

Thanks again for the query. I am here if you have any additional followup concerns as well.

Regards,

Dr. Galamaga

I guess my last question is that I have been extremely pale for the last few days aswell. My skin is almost yellow. It has becomethis way from a light brown complexion that Ihave. Could this also be due to stress/anxiety or should I go seek more medical advice?

Hello again,

The change in skin tone may be a reflection of stress. Check your eyeballs as well to make sure they are not yellow. A yellow appearance of the eye could reflect some inflammation of the liver. I really don't think you have any liver problem going as well so try not to worry too much about that or this perceived change in skin tone.

I really think you are dealing with a lot of stress and this can take quite a toll on your body. You will be fine and will continue to enjoy good health in the years to come. I am very confident of this.

Thanks again for the query. I hope this has served to be helpful for you. I am still here for you if you have additional concerns.

Sincerely,
Dr. Galamaga

I had low rdw and high monocytes. Mymonocytes have been elevated for a fewmonths just like this. What could this be due to. Should I go back to the doctor. Also my other question is if I had hep would this be revealed by the LFT?

Hello and thank you for the follow up question.

I have reviewed the test results which you have submitted and I see no significant concerning findings.

You have reflected some concern regarding the monocyte count as well as the RDW. RDW does not have anything significant in relation to your blood test. RDW is a measure of the relative width of the red blood cells. If you had some kind of anemia this would be something that would be significant. Right now this is nothing to worry about.

The elevated monocyte account also is not something I am concerned about. This is a small population of the white blood cells in your body and at this point it has nothing to do with the symptoms what you're experiencing.

Regarding hepatitis. There is nothing I see in your blood tests that indicate any active liver disease. You certainly have nothing to worry about along those lines. Perhaps there is some degree of anxiety which is somehow impacting the way you are feeling as I cannot see anything significant in your blood work at this point.

Viral hepatitis has nothing to do with any of these findings as well. I encourage you to review these results with your doctor who will I am sure you write what I am telling you here. Let me again reassure you that there appears to be nothing going on which is suspicious for any type of infectious activity or any type of disorder which is affecting your cardiovascular or gastrointestinal or blood subsystems.

Thank you again for your question and follow ups. I hope you found my response to be helpful reassuring and informative. If you have any additional concerns I would be happy to address them.

Sincerely,

Dr. Robert

I guess my last question is can Hepatitis be detected through a Liver functions test or is there a specialtest for it ?

It may seem that I have a high level of anxiety but my skin and eyes have turned yellow and my stomach hurts alot. But its either anxiety and stress causing this or Hepatitis. What do you suggest I do
doctor
Answered by Dr. Robert Galamaga (13 hours later)
Hello again,

"Hepatitis" literally refers to inflammation of the liver. If the hepatitis is active and caused by a virus or some kind of drug or alcohol there will usually be some abnormality in the liver enzymes. Viral hepatitis which is your main concern here, would usually have some liver function abnormality associated with it. We have great tests nowadays to detect viral protein and antibody to the virus so we can determine if someone has this infection with good certainty.

I realize you have some skin changes. Honestly though if the skin changes were due to liver problems, you would most certainly have some abnormal lab results. I understand why this would all cause you some anxiety and I hope I am reassuring you well that I believe you are healthy with no liver issue right now.

Thanks again for the query and followups. Let me know if I may provide any additional assistance.

Wish you good health.

Regards.

I guess I have been worying about the wrong things..but now I am extremely worried.

I have been itching for a few months and lately the Itching has gotten worst, my skin is yellow, eyes are not, abdominal pain, floating yellow stools, I read that all this is a sign of pancreatic cancer. I used to drink alot. I have quit but I still smoke although I am trying my hardest to quit. My doctor has ran cbc checks that revealed low hemoglobin but every thing else is normal. Liver function testarenormal and he tells me that my billirubin is normal so im probably just imagining my skin being yellow. Iknow what my skin looks like and it has definately turned slightly yellow. last week I went back to the doctor and told him to do something so he checked me for H pylori which cameback positive. I read that H Pylori can lead to stomach and pancreatic cancer.

I am real scarred because of my symptoms all point in that direction.
Are these signs for sure? Does H pylori cause jaundice or floating yellow stools?
Should I go to an emergency room and demand they do a CT scan? Could it be my gallbladder?

Im really scarred. I don't want to die at such an early age...but no one is hearing my cries for help via my physical signs....what should I do?

Hello and thank you for the follow up question.

First of all I would like to reassure you that I am very confident that you do not have any type of cancer at this time. I think you have been paying very close attention to your body recently which has made you extremely sensitive to any subtle changes. In essence this has caused a moderate degree of anxiety. Let me try to provide you with some reassurance here.

Regarding the testing which you have had performed the fact that you have normal liver studies as well as a normal blood count again supports the fact that you do not have cancer. The H pylori refers to a bacteria called helicobacter pylori. Over long periods of time. If somebody suffers from this type of infection it can increase the risk of development of different types of stomach cancer. The fact that you had this detected so quickly is very good for you. It can be treated very easily with an antibiotic combination. If you still have concerns regarding your stomach you could visit with a gastroenterologist. At some point he or she may consider doing an upper endoscopy where they use a small camera to look inside your esophagus and your stomach. I don't think this is a procedure which you need to have done right away at all.

If you would like you could to submit some pictures of your skin and I could take a look at these two further provide you with some reassurance about the fact that I am very very confident that you do not have any type of cancer or hepatitis. You can send these to my attention at the following e-mail address YYYY@YYYY

In addition I think it is extremely important that you concentrate on good lifestyle. You really need to focus on good physical activity every day in addition to a very well balanced diet. If you're having some anxiety I encourage you to explore the options including meditation as well as yoga. These are also good exercises for the mind and body. Finally is important that you discuss your concerns with friends and family so that they may continue to provide you with adequate support.

I thank you again for submitting your follow-up question. If you have any additional concerns as always I am here to help you.

Sincerely,

Dr. Galamaga

Hello and thank you for submitting your question and follow up.

Regarding the floating stools I would not be particularly concerned about this. I am sure this is related to your diet. In order to improve this. You might consider eating more fiber XXXXXXX foods. This includes vegetables as well as fruits. You might consider purchasing a fiber cereal such as shredded wheat. Also there are over the counter fiber supplements such as Metamucil which might also help with this issue.

I am very very confident that what you are dealing with right now is more a concern of moderate anxiety. At this time you have been tested for several different illnesses all of which have come back negative.

I am glad to hear that you have reported that you have a negative test for hepatitis although this is not surprising to me. I will await any additional information or pictures if you're interested in providing them.

Again I thank you for submitting your questions. I hope you have found my response is to be informative and reassuring. I await any additional concerns you might have.

Sincerely,

Dr. Galamaga
