Brief Answer:
PET CT Scan

Detailed Answer:
Hi

Enhancing soft tissue in the vallecula most likely lymphoid tissue the tissue has formed in response to the immune reaction in the body

Since you are having terrible neck and shoulder pain would advise you to do a PET CT scan to rule out anything serious such as cancer

Also you can take antibiotic for 1 week with your doctors advise and see if the symptoms persist.

If it persists then PET CT scan is a must.

If any space occupying lesion is found suspicious of malignancy in scan then biopsy test should be done to rule out cancer
