Question: I had some chest pain in August of 2022. I went to a cardiologist on Sept 2, 2022 and got a SPECT Exercise Treadmill Myocardial Perfusion done, which was abnormal, as you see from the attachment. I then went to another cardiologist who did anther test name PET Myocardial Perfusion and said it was normal on sept 14, 2022. I do have really bad GERD. What do you think of these 2 exams, based on them do you think I should repeat any of them? Is the first doctor right or was the second doctor right?

Brief Answer:
I would explain as follows:

Detailed Answer:
Hello,

I understand your concern and would explain that Spect is a less accurate test compared to PET for cardiac ischemia. So, I think that the second doctor’s result may be more correct and we can exclude myocardial ischemia.

Anyway, based on these test results and your clinical symptoms, we can not exclude a possible myocarditis.

For this reason, I recommend performing a cardiac MRI in order to examine better the myocardial tissue and possible signs of ischemia or inflammation.

This test would definitely clarify the cause of your complaints.

Hope you will find this information helpful!

I remain at your disposal in case of further questions whenever you need!

I was able to complete echocardiogram and stress echo and they said it was normal a few days ago. Do you still think I need an MRI since I have done the echo?

Opinion as follows:

Detailed Answer:
Hello again!

We can be more confident at excluding cardiac ischemia after these last tests. Anyways, if clinical symptomatology persists, cardiac MRI would be a good point for differential diagnosis (like inflammation, or alternative issues).

Please, let me know in case of any further questions!

Kind regards,
Dr. Iliri
