Should I Ask For Any Other Tests Other Than Hiv?

Question: Thank you very much Dr. XXXXXXX I have closed the question.
Just to continue one last question from the previous thread.
Lastly do you recommend that i should ask for any other tests other than hiv? knowing the fact that her eia changed from non reactive to reactive and the fact that she was falling sick.
They tested her for Hepatitis A, B and C at 5 months and 20 days after the exposure and they were negative.
My child was 3 years at the time of exposure.
Now she is 14 years old doing good except for the allergies are still there. .
Once again thank you very much for your precious time.

Brief Answer:
No need for further HIV test for your daughter.
Detailed Answer:
Hi,
Welcome back.
Transmission of HIV through a casual contact with blood has negligible risk only as per CDC guidelines. (https://www.cdc.gov/hiv/risk/estimates/riskbehaviors.html)
Your child does not need any more test for HIV infection as the Western blot test remain nonreactive for HIV even after 6 months of that incident and the HIV PCR tests for both HIV 1 and 2 were also negative.

Hi Dr.
Thank you. If it was a casual contact I would have never worried or even thought about it in my life.
It was a blood contamination incident wherein they kept the bloody tourniquet from the previous patient, the blood was on the hands of the person injecting to draw the blood from my kid's hands. It was an extremely unfortunate event and since the child’s hands are so small the blood came onto the elbow of my child. The person who was opening the butterfly felt the tip on to his gloves which had the blood and then drew the blood from the elbow of my child which already had blood. Unfortunately, the person doing it was an old guy and he is not even realizing. He again took the butterfly and again blood was dropping on the floor.
I even asked if there is anything I need to worry from the old patient record they did not tell anything.
Do you recommend that i should ask for any other tests other than hiv? knowing the fact that her eia changed from non reactive to reactive and the fact that she was falling sick just(allergies(mostly), cold, cough, and also fever) after the exposure as I told you earlier in the previous thread. The first year after the exposure was rough we had night mares, she was falling sick so many times. From the second year after the exposure things got better with mostly only allergies. She still has allergies 11 years after the incident but other than that she is good, growing healthy doing good.
Thank you Doctor.

Dr,
I forgot to include this that The most unfortunate thing was that BY the time i have realized and looked into this it was already too late, he has already put the butterfly into the vein. It happened so quick and i was talking about something else.

Brief Answer:
Tests proved that your daughter is free from HIV infection,
Detailed Answer:
Hi,
Welcome once again.
I can understand your concern now.
There is every chance for the previous patient whose blood was on tourniquet or over the gloves could be a HIV uninfected individual. Then there is no question of HIV transmission at all.
Persistent non reactive Western Blot tests and the PCR tests, it is sure that your daughter not get infected so far. So forget the incident and proceed with your normal routine.
There is no possibility for seroconversion here afterwards.

Yes Dr. XXXXXXX
I do agree with you that the previous patient blood could be a hiv uninfected individual.
I wouldn't have would have thought, Since she had got so much allergies after the unfortunate incident.
They prescribed my child the Albuterol, budesonide (oral inhalers) in her nebulizer and some antiboitics(if she has fever), she used to take Nasonex shots, and she uses Waldryl allergy and Zyrtec. This was in the first two years.
I did not know anything about these steroids affecting any HIV antibody test, so I used to give it to her regularly and I think even a day before the testings, since she was getting allergies a lot after the exposure.
Does her Nasonex inhaler(steroid) or budesonide inhaler(steroid) or sometimes QVAR have any effect on her Western blot test for HIV in the HIV Antibody test or the Hep A, Hep B, Hep C antibody test result? These were inhaled versions Dr. XXXXXXX

Brief Answer:
Inhalers and nebulizers won't affect the HIV antibody test results.
Detailed Answer:
Hi,
Welcome back.
Systemic steroids can delay the HIV antibody formation inside the body, but not more than 6 months. Inhaled steroids and the nebulizer would not interfere with antibody formation and the western blot test, that too even after 6 months. If you have still doubt, there is nothing wrong to go for the test once again. It cannot remain nonreactive even after 11 years in the presence of an infection.
Antihistamines (Zyrtec) and the antibiotics (act against bacteria and not having any effect on virus) will not have any effect on HIV antibody formation and so not affect the report of western blot test.
Her allergic episodes, spells of fevers would not have any relation to HIV. These can occur to anyone.

Thank you Dr. XXXXXXX
The steroids budesonide and some times the Nasonex or QVAR were inhaled versions, she used to inhale.
So does that affect any HIV western blot tests by causing a false negative test?
Can I let it go or do you think i should go for another test?

Lastly she has grown from 3 years to 14 years now and we are talking 11 years later. Would she have shown other severe symptoms other than the rough first 1 year and slightly upto 2 years after the exposure.
Some places even say that by 10 years most of them will have full blown symptoms.

Brief Answer:
Steroid inhalation won't affect the HIV report
Detailed Answer:
Hi,
Welcome back.
Once again I am affirming that steroid inhalers wouldn't affect the outcome of HIV western blot results.
Yes, you are correct, if she had HIV infection, by this time it would have reached advanced immune loss and their symptoms.
If you have another western blot test for your daughter and confirm with another nonreactive result, the you will be relieved from your unnecessary mental tension. Have a great day ahead.

I will try to let it go and move on. It took a lot of toll for us as parents in the past 11 years and going for one more test will make us more anxious. I will try to let it go and hope for the best.
Have a good day Sir.

Lastly when you say this in your reply If you have another western blot test for your daughter and confirm with another nonreactive result, the you will be relieved from your unnecessary mental tension. I think you mean if i go for one more HIV Antibody test (and not specifically Western blot) Sir? As you are mentioning if i get one more non reactive result?
Thank you.

Since in the reports they were mentioning as Western blot results as negative.

After 8 years Since we also had the other hiv 1 dna pcr qualitative and hiv 2 dna/rna pcr qual realtime tests and they were negative. These were the tests from Quest Diagnostics.
It says that PCR - HIV-1 DNA PCR is a sensitive method for detection of pro-viral DNA. It has been used in the management of perinatal HIV infection.
After all these tests and the years, do you still think there might be anything missing?
Do you think i can forget about this and move on?

Thank you for all you help Dr. XXXXXXX I will try to forget about it and move on. Since it is very stressful situation for us and already as parents we went through night mares for the past 11 years. Testing is also making us anxious.
If there is anything in the future i might need i will reach out to you. Since i see that you are an HIV/AIDS, STD specialist.
I also hope that this condition that she had might not be due to anything else other than hiv.
